DOI 10.20333/25000136-2023-6-5-15 Authors Annotation Th e main goal of therapy for the most acute phase of ischaemic stroke is to restore adequate cerebral blood fl ow through arterial recanalisation carried out using systemic thrombolytic therapy and/or endovascular surgical treatment. However, modern methods of reperfusion increase the risk of developing haemorrhagic transformation of the ischaemic focus, which can lead to an increase in neurological defi cit and an unfavourable outcome of the disease. Th erefore, the study of predictors of haemorrhagic transformation is important from the standpoint of developing methods for predicting its risk for patients before performing reperfusion therapy. A review of the literature sources contained in the databases of the RSCI, Google Scholar, PubMed, Scopus for the period from 1995 to 2023, considering possible risk factors for haemorrhagic transformation of ischaemic stroke, was carried out. Uterine fi broid is a benign hormone-sensitive proliferating tissue consisting of phenotypically altered smooth muscle cells of the myometrium. Th e genetic nature of this disease is of great interest. Th e aim of the research. Based on analysis of scientifi c literature, to identify the role of genetic mechanisms in formation and growth of uterine fi broids and the most common genetic variants of these tumours. Material and methods. Such websites as PubMed, eLibrary and CyberLeninka were used to search for the literatures to analyse. Th e articles selected were published within the period from 2017 to 2022. Th e combinations of keywords used for the search were: uterine fi broids, leiomyoma, genetics of uterine fi broids. Results. Currently, there is information regarding genes (genes of growth factors, of metabolism and reception of female sex hormones, cytokine genes, matrix metalloproteinase genes, genes of xenobiotic metabolism enzymes) the polymorphism of which is associated with an increased risk of uterine fi broids. However, in modern genetics, while identifying genetic predisposition to diseases, the analysis of individual specifi c genetic polymorphisms fades into the background. Currently, many studies are conducted using genome-wide research methods. Genome-wide research methods revealed three main genetic triggers of uterine fi broids: mutations in the gene of the mediator complex subunit 12 (MED12); mutations in the high-mobility group AT-hook 2 (HMGA2) gene; inactivation of the fumarate hydratase (FH) gene. In addition, against the background of uterine fi broids, there are cytogenetic changes in tumour cells. Th is is the basis of the “multi-stage” hypothesis of tumour development, according to which the function (or dysfunction) of a number of genes in multiple loci leads to tumour growth. Conclusion. Genetic predisposition plays an important role in development of uterine fi broids. Despite the signifi cant progress of modern medicine in diagnosis and treatment of uterine fi broids, the problem has not been solved yet requiring further investigation. To study the possibility of application of the “speckle tracking” technique for patients with isolated atrial septal defect in the preoperative and early postoperative period. To determine the dependence of myocardial strain parameters on the presence and the degree of myocardial chamber dysfunction. In search for the literatures, data published in the Medline, Embase, Cochrane Central Register of Controlled Trials (CENTRAL), Web of Science, Scopus and RSCI databases in the period from January 2004 to September 2023 were analysed. Material and methods. In the group of children with atrial septal defect (ASD), a technique for determining the strain in the myocardium of the right atrium (the cardiac chamber that is most hypervolemically compromised in this case). A comparison between the parameters of cardiac chamber strain and the routine morpho-functional parameters measured using echocardiography (EchoCG). Th e dynamics of myocardial strain parameters was assessed in the preoperative and early postoperative period in children with ASD and compared with the parameters of healthy children. Results. In the study, the parameters of myocardial strain in the left and right ventricles as well as the left and right atriums in children with ASD and in the comparison group of healthy children were determined. Th e comparability and constancy in the indices of myocardial strain of the ventricles among subjects of all studied groups. A signifi cant increase in longitudinal strain of the myocardium of the left and right atriums in the group of patients with septal defect in the preoperative period (in patients with indications to surgical treatment) in comparison with the studied patients in the rest of the groups. A decrease in the parameters of longitudinal strain of the left and right atriums in the early postoperative period aft er surgery for liquidation of interatrial communication has been reported. Conclusion. Th e parameters of global longitudinal strain of the right atrium diff er in the groups of patients with ASD in the preoperative and early postoperative period depending on the overstrain and dysfunction of the right atrium. Th erewith, we deem it necessary to form normative values for longitudinal strain of cardiac chambers in children (especially the right parts). To compare functional and structural disorders of the bronchopulmonary system in people of conscription age with undiff erentiated connective tissue dysplasia (uCTD) and malnutrition. Material and methods. A total of 60 male patients of conscription age from 18 to 27 years old were examined, of which 30 had phenotypic signs of connective tissue dysplasia, without acute or chronic bronchopulmonary pathology, while 30 age-matched volunteers did not have any apparent signs of health issues and were enrolled in the study as a control group. All patients underwent multispiral computed tomography of the chest organs (MSCT of the chest) with determination of density indicators in various parts of the lungs, spirometry with assessment of the functional state of the lungs and body plethysmography with the study of the structure of the total lung capacity with an assessment of bronchial resistance. Results. A comparative analysis of the parameters of the function of external respiration system showed that the ratio of FEV1/FVC,% and respiratory minute volume (RMV), regardless of the level of bronchial resistance, in patients with uCTD and malnutrition was signifi cantly higher than in the control group. Th e indicators characterising the structure of the total lung capacity (RV, TLC, RV/TLC) in patients with CTD were signifi cantly higher (p<0.001) than in the control group, and they did not depend on the values of bronchial resistance. In pa tients of the main group, a signifi cant decrease in lung density was found not only in the study of integral parameters, but also in the assessment of regional lung densitometric parameters. Conclusion. Analysis of the functional parameters of lung ventilation in patients with uCTD against the background of malnutrition showed signs of hyperventilation syndrome, pulmonary hyperinfl ation and primary pulmonary emphysema, the presence of which is also confi rmed morphologically by a signifi cant decrease in the integral and regional densitometric parameters of the lungs. Morpho-functional assessment of the state of the bronchopulmonary system with connective tissue dysplasia and underweight at the preclinical stage can be proposed for timely diagnosis (at the stage of the draft board) of primary pulmonary emphysema and prevention of the development of its severe complications. Identifi cation of socio-demographic characteristics and analysis of the structure of comorbidities in patients with stroke residing in the Arkhangelsk region. Material and methods. Th e results of treatment of 1,665 patients with stroke discharged from the regional stroke centre (RSC) of the Arkhangelsk Regional Clinical Hospital (ARCH) from January 2017 to December 2020 were analysed. We assessed the gender and age structure, the number of days before hospitalisation in the RSC from the onset of the disease, the distribution of patients depending on the type of stroke, and concomitant diseases. Results. Th e average age of the patients was 63.7±11.3 years. Men predominated in the age group of 41-69 years and women prevailed in groups of subjects aged over 70 years. In men aged 50-69 years, compared with women, atherothrombotic and lacunar strokes occurred more oft en, and in women, cardioembolic strokes occurred. In all age groups, except extreme ones, the number of cryptogenic strokes predominated in women. In 63.5 % of patients, from 1 to 3 concomitant diseases coexisted, in 33.7 % – from 4 to 6. Arterial hypertension (AH) was the most common one, isolated and in combination with other diseases; in men, it was more oft en combined with cerebral atherosclerosis (CA) or coronary heart disease (CHD); in women, combinations of hypertension with diabetes mellitus (DM) and chronic heart failure (CHF). Among all ages, women were more likely to have diabetes, atrial fi brillation (AF), angina pectoris (AP), and CA. In young and middle-aged men, hypertension, ischemic heart disease, postinfarction cardiosclerosis (PICS) and CHF were more common. Conclusion. It is necessary to take into account the combination of concomitant diseases in the patient with stroke, which will determine a patient-centered approach in secondary stroke prevention and rehabilitation. Th e identifi ed sex and age groups of patients with predominance of certain concomitant pathology can be taken into account when developing measures for the primary prevention of stroke. To reveal the occurrence rate of the Phe412Leu TLR3 gene polymorphism and to establish its contribution to the development of organ dysfunction in patients with severe pneumonia with A/H1N1 infl uenza. Material and methods. A total of 100 patients with pneumonia were examined. Of these, 45 patients with severe pneumonia, 55 with non-severe pneumonia. Th e age of the patients was 53 [37; 65] years. Men made up 48 % and women 52 % of the sample. Patients with severe pneumonia were divided into 2 groups: group 1 – with SOFA <2 points, group 2 – SOFA≥2 points. Determination of SNP genes was carried out via PCR using standard kits by “Lytech” (Moscow). Amplifi cation of the TLR3 gene fragments was carried out in a thermal cycler (Bis-M111 model, Bis-N LLC, Novosibirsk). Genomic DNA isolated from whole blood leukocytes using the “DNA express blood” reagent was subjected to analysis, then the amplifi cation reaction was carried out. Th e detection of amplifi cation product was performed in a 3 % agarose gel. Comparison of genotype frequencies in groups was performed using the χ2 test with Yates’ correction for continuity using a contingency table; if the expected events in one of the cells were less than 5, Fisher’s exact test was used. To assess the association of genotypes with the severity of the disease, the odds ratio (OR) was calculated with 95 % confi dence intervals (CI) according to genetic models (codominant, dominant, recessive and overdominant). Results. An association of TLR3 Phe412Leu gene genotypes with pneumonia has been revealed, according to codominant (OR (95 % CI) 0.438 (0.236–0.812) (p=0.013)), dominant (OR (95 % CI) 0.453 (0.255–0.805) (p=0.011)) and overdominant (OR (95 % CI) 0.503 (0.279–0.906) (p=0.032)) genetic models. At the same time, in patients with severe pneumonia against the background of A/H1N1 infl uenza, the risk of organ dysfunction increases (Group 2 (SOFA≥2)) when carrying the -412Leu allele, according to the recessive inheritance model (OR (95 % CI) 9.200 (1.004–84.262) (p=0.042)). Conclusion. Th e severe course of pneumonia in infl uenza A/H1N1 is accompanied by a high incidence of organ dysfunction. To study the frequencies of genotypes and alleles of polymorphisms rs6737848 of the cytokine signalling suppressor gene (SOCS5) and rs2227983 of the epidermal growth factor receptor gene (EGFR) within individuals with allergic bronchial asthma (ABA), as well as an assessment of the relationship between the studied polymorphisms and bronchial asthma. Material and methods. Th e study included 179 patients with ABA and 217 individuals in the control group. Th e control group consisted of people without ABA, comparable with the main group by the gender and age. Patients with ABA underwent standard laboratory and instrumental examinations, as well as DNA typing. Molecular-genetic analysis of the rs6737848 polymorphism of the SOCS5 gene and rs2227983 of the EGFR gene was conducted using the polymerase chain reaction method. Results. Among individuals with ABA, including women, the CC genotype and C allele of rs6737848 of the SOCS5 gene were signifi cantly more prevalent compared to the control group. Th e risk of developing ABA, evaluated by odds ratio (OR), was 2.1 times higher (95 % CI 1.266-3.490, p<0.05) in carriers of the CC genotype of the SOCS5 gene compared to carriers of the CG and GG genotypes. A signifi cant predominance of individuals with the heterozygous AG genotype of the EGFR gene was found in the ABA group (48.6 %) compared to the control group (36.9 %, p<0.05). It has also been noted that this genotype of the EGFR gene was more frequently observed in women with ABA compared to the control group (53.4 % versus 32.7 %, p<0.05). Conclusion. Th e results of the study have made it possible to assess the association of the polymorphism rs6737848 of the SOCS5 gene and rs2227983 of the EGFR gene with ABA. Th e CC rs6737848 SNP of the SOCS5 gene and AG rs2227983 genotype of the EGFR gene increase the risk of developing ABA. DOI 10.20333/25000136-2023-6-61-68 Authors Annotation Th e aim of the research. To study the peculiarities of the spread of metabolic syndrome (MS) and its components in the population of young men and women living in the Khanty-Mansi Autonomous Okrug – Yugra. Material and methods. A cross-sectional single-center case-control study of 863 non-indigenous and indigenous young subjects living in urban and rural areas with signs of metabolic syndrome was conducted. Results. Th e occurrence of metabolic syndrome among young people of Khanty-Mansi Autonomous Okrug – Yugra was 31.1 % among men and 44.0 % among women. Th e most common components of the metabolic syndrome in the examined young people were hypertriglyceridemia (68.1 %), hypercholesterolemia (46.1 %) and low-density lipoprotein hypercholesterolemia (48.9 %). High direct correlations were found between waist circumference and the level of total cholesterol, triglycerides in males (r=0.802 r=0.773) and in females (r=0.938 and r=0.951). Arterial hypertension is common in indigenous males residing in the rural areas (53.3 %). Conclusion. Th e metabolic picture of young subjects with MS is represented by a predominant combination of abdominal obesity and dyslipidemia, hypertriglyceridemia in particular. Th e high occurrence of metabolic disorders that form MS determines the need for comprehensive examination of young people in order to timely identify subjects at increased risk and carry out preventive measures aimed at reducing the development of cardiovascular diseases, diabetes mellitus and related complications. To determine the degree of frustration in children of the fi rst years of schooling and to evaluate the eff ect of antibodies to the brainspecifi c protein S100 in the release-active form (PA-AT S100) on the level of school anxiety. Material and methods. An observational study with a prospective evaluation of the results was carried out in the conditions of a typical school in Krasnoyarsk in the period March-May 2021. Taking into account the information of parents about increased anxiety, children were surveyed using the Philips test, which allows to study the level and nature of anxiety in primary school-age children, followed by an assessment of the eff ectiveness of the therapy. Th e sample of children had no gender diff erences, the median age of the surveyed was 8.9 (8.4; 10.4) Results. Th e performed study confi rmed the eff ectiveness of the therapy with an anxiolytic drug and showed positive dynamics with a reduction in the frequency of signs indicating behavioral disorders and a decrease in the level of anxiety in the absence of any adverse reactions. Conclusion. Th us, the present study showed the restoration of the general emotional state of the child and the improvement of the child›s adaptive capabilities to school life against the background of a decrease in general anxiety. Along with this, a decrease in the frequency and severity of negative emotional experiences associated with the fear of self-expression and an increase in the ability to self-disclosure and demonstrate one›s own capabilities were revealed. To investigate the prevalence of dyslipoproteinemia (DLP) in a large industrial region (Kuzbass) among the urban and rural population and to establish its associations with traditional risk factors and infrastructure parameters. Material and methods. Th e study included 1,598 respondents living in the city of Kemerovo and adjacent villages. Adverse infrastructure parameters were determined on the basis of patients’ answers to the NEWS (Neighborhood Environmental Walkability Scale) questionnaire. Th e lipid metabolism disorder was defi ned as a deviation from the normal range of any lipidogram parameter: total cholesterol (TC) >5.0 mmol/L, low-density lipoprotein cholesterol >3.0 mmol/L, high-density lipoprotein cholesterol (HDL-C) <1.0 mmol/L in men and <1.2 mmol/L in women, triglycerides >1.7 mmol/L or their combination. Obesity was diagnosed based on a BMI of ≥30 kg/m2 and abdominal obesity based on a waist circumference >94 cm in men and >80 cm in women. Results. Th e prevalence of DLP in Kuzbass was 87.0 %, with the highest occurrence of low-density lipoprotein fraction at 70.3 %. Among town residents, elevated TC levels were more common among women (70.1 %) compared to men (60.2 %), p=0.001. Similar data were obtained among rural residents with regard to reduced HDL-C levels: 31.1 % vs. 22.1 % (p=0.040). Th e traditional risk factors: age (OR=1.04; OR=1.02), arterial hypertension (OR=2.93; OR=2.21) and abdominal obesity (OR=3.29; OR=1.83) were associated with impaired lipid metabolism in urban and rural areas, respectively. Such infrastructure parameter as inaccessibility of cultural and recreational facilities was associated with the development of DLP (OR=1.47). Th e following non-traditional factors were associated with the development of hypertriglyceridemia: remoteness of the pharmacy (OR=1.37), longer walking time to the bank (OR=1.43) and distant grocery location (OR=1.40). In obese patients, the following adverse components were associated with lipid metabolism disorders: remote location of the fruit store, bank, pharmacy, public transportation stop and place of work. Conclusion. Th e present study has proved that behavioral risk factors mediated by unfavorable infrastructure parameters in a particular region have confi rmed high impact on one of the traditional risk factors for cardiovascular disease: DLP. Review. 2023;(6):84-88.. DOI 10.20333/25000136-2023-6-84-88 Authors Annotation Th e aim of the research. To assess long-term outcomes of a new surgical method for treatment of pilonidal coccyx cyst. Material and methods. Th e main group was formed from 32 patients who received standard and generally accepted treatment against PCC in Russia. Th e comparison group included 31 patients who received proprietary treatment against PCC (patent RU 2 760 094). Th e groups were of comparable age and diagnosis on admission. Th e evaluation of the results was compared in the long-term period. Results. As a result of using the new method, the time of wound epithelisation, inpatient treatment and the the patients’ incapacity to work decrease, the number of postoperative complications is reduced, the quality of life is signifi cantly improved as well as the cosmetic result of the surgery. Conclusion. Th e new surgical treatment method improves quality of life and reduces the risk of recurrence. To study in-hospital and long-term outcomes in patients with myocardial infarction aged over 90 years depending on the management strategy. Material and methods. Design-wise, the work was a retrospective single-centre study. Within the period from 01.2013 to 12.2020, 183 patients in the age group of 90 years and older were hospitalised with MI in the Samara Regional Cardiological Dispensary. Th e mean age was 91.7 years (90-100), 71.6 % women. Th e patients were divided into two groups depending on the chosen management strategy: conservative or invasive. Group 1 included patients receiving optimal drug therapy (n=146), mean age 91.8 (90-100) years, 107 (73.3 %) women. Group 2 included patients who, in addition to optimal drug therapy, underwent CG and, if indicated, PCI (n=37), the mean age was 91.1 (90-96) years, 24 (64.9 %) women. Results. Th e groups were comparable in clinical characteristics. In-hospital mortality among patients with MI aged 90 years and older was higher in the conservative strategy group compared to the invasive strategy group: 36.3 % versus 10.8 % (p = 0.003). One-year mortality within a year aft er discharge from the hospital was 30.8 % (45 patients) in the conservative management group and 32.4 % (12 patients) in the invasive management group (p=0.85). As a result, 98 patients (67.1 %) died in the optimal drug therapy group, while 16 (43.2 %) died in the invasive treatment group (p = 0.007). Life expectancy was 103.75 days for patients in the invasive approach group and 122.5 days for the conservative strategy group (p=0.57). Conclusion. To carry out a comparative analysis of coagulatory haemostasis indicators according to the “thrombodynamics” test in the postoperative period in patients with malignant glioma (MG) of the brain. Material and methods. Th e study included 20 patients with MG (13 patients with GB; 7 patients with grade 3 MG). Th e inclusion criterion was the absence of data on the venous thromboembolism. Th e study of coagulation hemostasis disorders was carried out using the “thrombodynamics” test 4 to 6 weeks aft er MG resection. Results. Hypercoagulation aft er MG resection was diagnosed in 9 (45 %) of 20 patients using the “thrombodynamics” test. A high level of blood D-dimer was found in 6 (30 %) of the 20 patients included in the study (median value: 1.065 (0.59-3.33) μg/ml). Only 4 (44.4 %) of 9 patients, diagnosed with hypercoagulation according to the “thrombodynamics” test, also had an increase in the level of blood D-dimer. Th e risk of detecting hypercoagulation signifi cantly increased only in the case of an increase in the blood D-dimer level of more than 0.5 μg/ml (RR 2.250; 95 % CI: 1.084-4.617; p=0.042). Conclusion. In half of the patients without venous thromboembolism, hypercoagulation occurs within 4 to 6 weeks aft er resection of MG according to the “thrombodynamics” test. Th e risk of hypercoagulation signifi cantly increases in patients with high blood D-dimer levels (more than 0.5 μg/ml). Analysis of the dynamics of the performance indicators of the Centre for Disaster Medicine and Emergency Medical Care (CDM and EMS) to assess the eff ectiveness of measures to optimize the provision of emergency medical care (EMS) to the population served for 2018-2022. Material and methods. А non-selection study was performed with mathematical and statistical analysis of primary electronic accounting and reporting documentation in the unifi ed medical information and analytical system “Promed” of the CDM and EMS, and forms of federal statistical observation №№ 30, 55, 56 for the analysed period. Results. Th e eff ectiveness of measures to optimize the regional model of the provision of EMS is demonstrated on the example of CDM and EMS. It has been shown that, as a result of the introduction of GLONASS/GPS satellite monitoring with visualisation and the creation of a unifi ed dispatch service (EDS), the time parameters of work of EMS teams have improved. As a result of improving continuity with emergency medical care divisions (EMCDs) of polyclinics, the proportion of forwarded calls increased (by 12-16%), the total number of visits decreased (by 28 % in 2022 compared to 2018) as well as the average daily load on EMS teams (by 4.8 %). Th ere was an annual increase in the number of calls to medical consultants of the operational department of the CDM and EMS by 1.1–1.2 times, which in turn aff ected the decrease in the number of unwarranted calls for EMS teams. Conclusion. Th e measures to optimise the regional model of the EMS have shown their effi cacy and made it possible to ensure the availability and timeliness of EMS provision to the population served in emergency and urgent forms. At the end of 2022, for the fi rst time, the indicator of the provision of the EMS approached the standards (290) and amounted to 312.9 calls per 1,000 population. DOI 10.20333/25000136-2023-6-107-111 Authors Annotation Th e article presents a clinical case of surgical treatment of a patient with acute destructive pyelonephritis with application of hybrid surgical technology at the premises of the urological department at the Krai Government-owned Publicly Funded Health Care Institution «Regional Clinical Hospital» in the city of Barnaul. Th e blood circulation status in the kidney was analysed using CT perfusion of the kidneys, the degree of endothelial dysfunction parameters correction and morphological analysis of the material. Th e postoperative period was favourable with preserved kidney, decreased hospitalisation term, lowered intensity of the pain syndrome in the area of aff ected kidney, positive dynamics of laboratory parameters in patients of the main group. Th e study results show the prospect in assessment of the approaches to treatment tactics for patients with acute destructive pyelonephritis based on the results of CT perfusion of kidneys. Th e presented method demonstrates the advantages of applying the hybrid surgical approach to treatment of patients with severe forms of destructive pyelonephritis. DOI 10.20333/25000136-2023-6-112-116 Authors Annotation Amyloidosis is a large group of diseases based on stromal-vascular proteinosis: extracellular deposition of pathological non-soluble glycoprotein in organs and tissues, which may lead to severe organ dysfunction. AL-amyloidosis incidence amounts to 8-12 cases in 1,000,000 of the population per year. Despite the fact that amyloidosis oft en occurs in clinical practice, this problem is unfortunately denied attention from Russian scientifi c societies, periodicals and physicians who, even with their experience and high qualifi cation, do not diagnose this disease. Th e review considers a clinical case of primary amyloidosis with predominant impairment of heart and the only kidney. Clinical picture-wise, heart and kidney failure was in the foreground. Despite the medicinal treatment provided, the patient’s condition was rapidly deteriorating. Th e diffi culty of accurate diagnosis was associated with the variety if the patient’s complaints and clinical manifestations, which lead to the verifi cation of the diagnosis post mortem. Modern diagnosis of amyloidosis requires biopsy of organs that are most vulnerable to this disease with the aim to detect amyloid fi brils in the aff ected tissues.
