Th e relevance of brachial plexus and peripheral nerves injury is displayed in the modern aspect. Th ere is data on variant anatomy of brachial plexus, maco-/ microscopic and intra-beam structure. Th e article provides historical reference, indicating the main stages in the development of peripheral neurosurgery from the moment when the fi rst suture was applied to the injured nerve, till the development of microsurgical techniques and neural transmissions. Material is presented on the structure of thoracodorsal nerve, methods of its use and eff ectiveness in modern concept of surgical treatment of injured nerves in brachial plexus. It covers in details scientifi c and practical use of walking video analysis at the present stage of medicine development indicating corresponding nosological units. At the beginning of scientifi c review there is a brief historical essay on the main stages of walking instrumental diagnostics formation and development in digital and pre-digital era of mathematic data processing. It also describes the most advanced technologies for obtaining biomechanical data using various computer models that are established in modern medical science. The history of these computer models appearance, their advantages and disadvantages, ways to correct these shortcomings are disclosed in the main part of the scientifi c review. Th e mentioned part of the review is devoted to the problem of kinematic data objectivity obtained by systems of movements video analysis using the technology of refl ective markers. Based on literature data, the author identified factors that aff ect the objectivity of the research results; there is assessment of infl uence degree of each of the factors; and also, kinematic indicators that slightly differ in diagnostic reliability are presented. Rahman graduate student; Kerala Institute of medical Sciences; dr.reenum@gmail.com Vijayaraghavan Govindan Dr.Med.Sci., Professor; Kerala Institute of medical Sciences; drvijaraghavan@gmail.com Annotation Th e aim of the research is to assess standard clinical parameters of patients, living in the southern region of India, with ischemic heart disease (IHD) against the background of primary manifested hypothyroidism; to identify possible correlations of parameters of fi rst level lipid profi le with the use of hormone replacement therapy (L-thyroxine). Material and methods. Th e study included 344 patients with ischemic heart disease of II-IV functional class (CCS, 1976). All patients included in the study gave written consent for it. Th e study group included 100 patients with IHD on the background of primary manifested hypothyroidism; comparison group included 244 patients with IHD without hypothyroidism. IHD was diagnosed on the basis of coronary angiography. Myocardium echocardiographic study was performed to assess morpho-functional parameters. Standard laboratory and instrumental clinical parameters were analyzed. Th e diagnosis of hypothyroidism is confi rmed by determining the level of thyroid stimulating hormone (TSH), free triiodothyronine (T3) and free thyroxine (T4). Ultrasound examination of thyroid gland was performed to determine the structure and to conduct diff erential diagnosis. Comparative analysis of clinical data was carried out, as well as regression analysis of the studied parameters infl uence for the prognosis of heart failure during two-year observation. Results. Th e study revealed signifi cant diff erences in body mass index (BMI), lipid profile, and thyroid hormone levels. An increase in myoglobin level and statistically signifi cant decrease in glomerular fi ltration rate (GFR) level were also found in the study group. When constructing a regression model, statistically signifi cant relationships between the lipid profi le and the use of replacement therapy were identifi ed; they indicate positive effect of L-thyroxine on lipid metabolism. Conclusion. Th e use of L-thyroxine in patients with ischemic heart disease on the background of primary manifested hypothyroidism does not only effect hormone replacement therapy, but it also has positive eff ect on the course of dyslipidemia. Material and methods. A simple comparative study included 76 comorbid patients treated in TB dispensary for tuberculosis, who had COPD and AH at the same time. All patients had hypertension, stage II, and were divided into 4 groups (19 people in each group); each group received a certain combination of antihypertensive drugs. Group 1 – angiotensin-converting enzyme inhibitors (ACEI) + diuretic (D) 1 time per day for 3 months; Group 2 – angiotensin receptor antagonists (ARA) + diuretic (D) 1 time per day; Group 3 – ACE inhibitors + calcium antagonist (AK); Group 4 – ARA + AK. Th erapy eff ectiveness check-up was in the form of daily blood pressure monitoring (DBPM) (at the beginning and in 3 months aft er therapy beginning). Results. By the time of groups inclusion into the study, they were comparable by gender, nature of basic and concomitant pathology. In 3 months of therapy, certain degree of decrease in average systolic, diastolic and pulse blood pressure was obtained in all combinations according to the results of DBPM. The best antihypertensive eff ect with minimal severity of side eff ects was obtained in ARA + AK group. Drug tolerance is considered as satisfactory. Th e exception was patients with cough on the background of taking ACEI; in one case, drug withdrawal was required. Conclusion. Hypertension treatment in comorbid patients with TB and COPD using ARA and AK allowed achieving the best results in stabilizing hypertension. Material and methods. 65 patients with COPD aged 45-60 years treated in pulmonological hospital were examined. Patients are divided into 4 groups depending on the frequency of COPD exacerbations and the severity of clinical symptoms. Along with standard examination methods, kidney function was evaluated by studying the level of urea, creatinine, cystatin C in blood serum; glomerular fi ltration rate was calculated based on serum concentrations of creatinine and cystatin C; the level of daily albuminuria was also studied. Results. Exceeding urea normal values was observed in 21.5% of patients. People with frequent exacerbations of COPD and severe symptoms had higher level of urea and lower glomerular fi ltration rate compared to patients with low incidence of exacerbations and mild clinical manifestations. Th e increased albuminuria level was observed in 29.2% of cases and mainly in people with high frequency of COPD exacerbations. Chronic kidney disease of stages 1 and 2, characterized by the preserved glomerular fi ltration rate, was found in 21.5% of patients, and of stage 3 – in 16.9% of the examined. Correlation analysis showed interconnection of the studied indicators of renal function and frequency of COPD exacerbations, data from clinical CAT and mMRC tests, blood leukocyte level, smoking person index, age and body mass index. Conclusion. Material and methods. 46 patients with acute myocardial infarction were examined, their sulfated glycosaminoglycans blood serum content (sGAG) was studied in dynamics; verifi cation of remodeling type was carried out due to echocardiography. Results. In case of hypertrophic type, an increase of sGAG content was determined in all reparative fi brosis periods. In patients with dilated remodeling type, a slight increase of sGAG was recorded in the second period. In case of mixed remodeling type, gradual increase of sulfated glycosaminoglycans blood serum content was registered. Conclusion. Changes in the level of blood sulfated glycosaminoglycans in patients with hypertrophic remodeling type are likely to lead to faster scar formation and prevention of left ventricle dilatation. In dilated type, there is “slower” scar formation with subsequent heart chambers dilatation or formation of postinfarction LV aneurysm. It can be assumed that a lower sGAG level in period I in patients with mixed remodeling type (compared with period I in hypertrophic type) initially forms inferior connective tissue scar in such patients, which can lead to left ventricle dilatation. Voino-Yasenetsky Krasnoyarsk State Medical University; neurodmit@mail.ru Annotation The aim of the research is determination of threshold sensitivity of olfactory perception in people with extrapyramidal diseases and improvement of protocol for conducting this examination to exclude all possible error sources and misrepresentation of measured parameters that may arise due to the characteristics of the examination protocol itself. Material and methods. 45 patients with PD, 40 people with ET, and 64 conditionally healthy people were tested. Assessment of olfactory function was carried out by means of Sniffi n’s sticks test (“Burghart Messtechnik”, Germany), which includes 3 subtests determining the following indicators of olfactory perception: threshold sensitivity, discriminatory sensitivity and identifi cation of a specifi c smell. IBM SPSS Statistics 23 program was used, as well as Microsoft Excel for data analysis. Statistical processing of the data was performed using Shapiro-Wilk, Student, Mann-Whitney criteria and Spearman’s correlation coeffi cient, as well as non-linear method of elastic maps. Elastic maps were created by means of ViDaExpert. Results. It was determined that the results of threshold sensitivity determination according to original protocol are equally bad both in sick people and in a group of healthy ones. Th ere appeared a hypothesis that it happens due to the current study protocol. In this regard, a new testing methodology was proposed, which involves presentation of triplets with a smell in various concentrations in a random order, rather than increasing concentration of odorous substance from less to more. New examination protocol was tested on a group of healthy people and the obtained results showed no statistically signifi cant diff erence from those obtained using the original technique, however, for finalization of recommendations it is necessary to conduct such study on a group of sick people Conclusion. It is important to implement Sniffi n’s sticks test as a method for early diagnosis of PD, as well as a method for diff erential diagnosis of PD and ET. However, the infl uence of data collection procedure concerning olfactory function on the study result was discovered, that should be taken into consideration when using this method. Material and methods. Th e study included 107 patients diagnosed with prostate cancer from high progression groups. Th ese are groups of patients who underwent surgical treatment with/without adjuvant radiation therapy. Furthermore, cohorts of patients with focal spread of extra-prostatic invasion, as well as patients with diff use spread of extra-prostatic invasion and invasion into seminal vesicles were additionally identifi ed from each group. Comparison of the studied groups of patients in terms of survival using Kaplan-Meier method was made. Results. In group of surgical treatment as monotherapy, the 8-year overall survival rate was 66 %, progression-free survival rate was 55 %, while cancer-specifi c survival rate was 82 %. In group of surgical treatment with radiation therapy, the 8-year overall survival rate was 84 %, progression-free survival rate was 70 %, and cancer-specifi c survival rate was 93 %. When comparing within these groups patients with focal prevalence of extra-prostatic invasion, no statistically signifi cant diff erence was revealed – overall survival p = 0.535, progression-free survival p = 0.198, cancer-specifi c survival p = 0.328. Conclusion. The use of surgical method combined with radiation therapy improves overall and cancer-specifi c survival rates. Th e focal spread of extraprostatic invasion is not an indication for adjuvant radiation therapy. Material and methods. Th e study included 60 patients with local cold injury of lower extremities of III – IV degree. Th e victims were divided into 3 groups depending on the level of tissue injury. Blood sampling was carried out in the late reactive period and in the period of granulation and epithelization. Calprotectin level was measured by means of multiplex blood test with a set of reagents from Biomedical. Determination of lymphocyte-platelet adhesion (LPA) was carried out according to the method proposed by Yu. A. Vitkovsky et al. (1999). Th e LPA index was calculated as a number of LPA per one hundred of lymphocytes. Th e adhesion level was evaluated as the average number of platelets adhered to lymphocytes. Results. In the studied groups of patients with frostbite of lower extremities, an increase in serum calprotectin was recorded. Calprotectin level is directly proportional to the volume of aff ected tissues. It was found that in patients with local cold injury, the percentage and lymphocyte-platelet adhesion level increases. Th e activity of lymphocyte-platelet interactions increases with the increase in the volume of aff ected tissues. Conclusion. Th e revealed increase in serum calprotectin level in patients with local cold injury is probably connected with leukocytes activation and their degranulation. Activated forms of white blood cells interact with blood platelets, lead to enzymatic explosion in alteration focus and perifocally, provoke endothelial dysfunction. Th e ongoing and growing phenomenon of lymphocyte-platelet interactions create foundation for the beginning of repair, penetrating into the interstitial space. Th e established changes in serum calprotectin increment and intercellular interactions are directly proportional to the volume of the affected tissues. Material and methods. Th e study included 166 patients, including 82 (49.4 %) males, 84 (50.6 %) females. All the patients included in the study were operated on, followed by morphological study of the removed drugs. Th e patients were divided into two groups. Th e control group (69 patients), whose perioperative management was in accordance with standard methods, were selected retrospectively. The main group consisted of 97 patients whose perioperative management was in accordance with accelerated rehabilitation protocol (developed and implemented in the clinic since 2014). Results. Th ere were no statistically signifi cant diff erences among patients in the compared groups by age, gender, ASA status, or body mass index. No statistically signifi cant diff erences were found between the two groups in terms of average duration of operation, intraoperative blood loss, and frequency of various surgical interventions. No statistically signifi cant diff erence was found in the frequency of complications (p=0.5), as well as their severity (p=0.3) in the compared groups. Gastrointestinal tract function restoration was signifi cantly faster in the main group compared with the control group (p=0.01). The average postoperative hospitalization period was 10.7±7.1 days in the main group, 16.2±13.2 days in the control group (p=0.005). Conclusion. Mutations in CYP1B1 gene are the most common cause of the disease. Th e present paper presents a clinical case of two siblings who are mutation carriers of c.1330 C> T p. (Arg444Ter, rs377049098) and c.1331 G> A p. (Arg444Gln, rs72549376) of CYP1B1 gene in compound heterozygous state. The study allows to suggest that simultaneous carriage of the two detected variants can determine malignant nature of the course of the disease. Voino-Yasenetsky Krasnoyarsk State Medical University; happyponchik-2@mail.ru Annotation Treatment of severe forms of adenomyosis combined with hysteromyoma is one of the most difficult and disputable problems in modern gynecology. Th e article presents our experience in performing organ-preserving operations in patients with hysteromyoma combined with diff use adenomyosis, as an alternative to hysterectomy. Four our own clinical cases as an example, demonstrate the eff ectiveness of complex treatment of patients with diff use adenomyosis combined with hysteromyoma. During the period of 2012-2019 we performed surgical treatment of 41 patients with diffuse adenomyosis, while it was noted that in 51,2 % of cases (21) diff use adenomyosis was combined with hysteromyoma. Th e example of four clinical cases demonstrated the possibility of successful treatment of this complex pathology in females of reproductive age, which allowed them not only to avoid organ-removing surgery, but also to eliminate severe symptoms of the disease, as well as to maintain the possibility of further child-bearing. Organ-preserving surgical treatment of diff use adenomyosis and hysteromyoma according to Hisao Osada method is a possible and eff ective alternative to classical hysterectomy. Even with a typical onset of the disease, it is not always possible to diagnosis it correctly and timely. There are no unifi ed approaches for intensive care of such patients. Th e article describes a clinical case of traumatic rhabdomyolysis with atypical onset of the disease. Complex intensive care included permanent renal replacement therapy with sorption elements in combination with hyperbaric oxygenation. Th e success of treatment allows to consider the applied technique promising in treatment of patients with severe traumatic rhabdomyolysis. Material and methods. Quality indicators developed by us in the context of “сase мanagement” program realization. Results. “Case management” program, being constantly improved in real time, made it possible to generate patient steams, to reduce examination time, to minimize unreasonable consultations, to regulate timely the queue to specialists, and to decrease examination time for patients from 21 days in 2017 to 7 days in 2018 and to increase patient satisfaction from 58.2 % to 94.1 %, respectively. Voyno-Yasenetsky Krasnoyarsk State Medical University; vinogradovl6@yandex.ru Annotation The aim of the research is the assessment of possible coronavirus infection (COVID-19) spread by means of mathematical modeling and its consequences in Krasnoyarsk city. Material and methods. Data on the total Krasnoyarsk city population and number of daily detected cases of coronavirus infection starting from the fi rst detected case are used. Mathematical model published by Z. Liu et al., (2020) was used as a model of epidemic coronavirus infection, COVID-19 process. The main feature of this model is that preliminary calculations allow the use of model at the initial stage of infection spread in certain area. Results. According to the results of the assessment of possible coronavirus infection COVID-19 spread in the territory of Krasnoyarsk city, in case of no external infl uence such as quarantine or some other activities, infected by COVID-19 patients’ peak would be on 65–70 days dating from the time the first patient was identified. In this regard, the number of patients within this period would exceed 200,000 people, while 46,000 patients would remain unknown to health care system. Conclusion. The obtained modeling results of coronavirus infection COVID-19 spread in Krasnoyarsk city revealed that in case of no infl uence on epidemiologic process, potentially, in 2–2.5 months since the fi rst infected patient was detected, a large number of patients could be expected. In case of significant increase in number of patients with coronavirus infection, COVID-19, the number of patients who need hospitalization and intensive care would increaseas well.
