Genetic variations of transporter gene of the solutes 1B1 (SLCO1B1), which encodes a protein OATP1B1, may affect to the pharmacokinetics of drugs and lead to interindividual variation in the response of the body to the received pharmaceuticals. In this article we will discuss current knowledge about expression, functioning of the protein OATP1B1 and the impact of gene polymorphisms SLCO1B1 to the efficacy and safety of various drugs. The importance of immunohistochemical markers for the prediction of clinical course and outcome of atypical hyperplasia and endometrial adenocarcinoma is discussed. Several studies proved that some markers can be independent prognostic indicators. Listed in the scientific review data allow the rational approach to the treatment of these patients. Voyno-Yasenetsky Krasnoyarsk State Medical University; allasalmina@mail.ru Annotation The aim of the research.To study the regulatory role of connexin 43 (Cx43) in intercellular interactions in the neurovascular unit (NVU) in vitro. Materials and methods.The research was conducted on a model NVU in vitro with the evaluation of Cx43 and CD38 expression in physiological conditions and in the suppression of Cx43 expression. Results. NVU cells-components in vitro express Cx43 and CD38. Application of siRNA reduces the expression of Cx43 and CD38 on neurons and endotheliocytes. There is a direct correlation of medium strength between expression of Cx43 and CD38. Conclusion.During the work it was established that there is functional conjugation between the molecule CD38 and Cx43, distinguished by their characteristics in NVU cells and indicated of their different sensitivity to the action of factors that violate the metabolism of NAD +. Ammosov; tyanic@mail.ru Annotation The aim of the research.To assess the state of the microcirculation in the lower limbs using transcutaneous oximetry in patients with predominantly sensory forms of chronic inflammatory demyelinating polyneuropathy (CIDP). Materials and methods.The study involved 57 people with sensory variant of CIDP at the bas of the University Hospital KrasSMU named after Prof. V.F. Voino-Yasenetsky. The age of patients ranged from 12 to 75 years old, the age median was 28 years old. Methods of diagnosis: clinical neurological, neurophysiological. Results.During the transcutaneous oximetry using the equipment «Radiometer TC 4 series» (Copenhagen, Denmark) in patients with chronic inflammatory demyelinating polyneuropathy were predominated, together with normative indices, compensated disorders of tissue metabolism. It was combined with the clinical manifestations of the disease and reflects the degree of angiotrophoneuritic syndrome. Conclusion.Transcutaneous oximetry allows to verify the microcirculation disorders due to involvement nervi vasorum at chronic inflammatory demyelinating polyneuropathy (CIDP). Analysis of the frequency of genotype polymorphism rs231775 gene STLA4 in patients with asthma, residents of Krasnoyarsk, as well as assessing the relationship of the studied polymorphisms and asthma. Materials and methods. Genomic DNA was isolated from 10 ml of venous blood by phenol-chloroform extraction. DNA testing was conducted using the polymerase chain reaction in real time. A group of patients with bronchial asthma - 100 persons, the control group - 338 persons. Results.It was studied the frequency of polymorphism rs231775 gene CTLA4 separately in patients with allergic and nonallergic bronchial asthma (BA). A comparative analysis revealed that in patients with allergic asthma the frequency of genotype G / G was 1.7 times higher as compared with the control group and was amounted in allergic asthma patients - 36.4% in the control group -21.9% (p = 0.031). Frequency of genotype G / G and A / A + A / G polymorphism rs231775 CTLA4 gene in AD patients was 36.4% and 63.6%, while in the control group - 21.9% and 78.1% respectively. Conclusion.In the study of frequency polymorphism rs231775 gene CTLA4 was revealed the increasing the frequency of genotype G / G in the group of patients with allergic asthma. Considering these results, we can conclude that the genotype G/G (OR = 2.036; 95% CI - 1,16-3,58, r1-3 = 0.012) determine increased risk of allergic asthma. Materials and methods.Examined by solid phase ELISA of cytokine concentrations in serum and immunoglobulin in 57 patients with chronic rhinosinusitis polypous. Results. It was found the increase of concentration of IL-8, -5, -4, -10, IgG, M, E and sIgA in the majority of patients. Conclusion. Determination the concentrations of cytokines and immunoglobulins is important for its use as a diagnostic and prognostic criteria of origin, course and recurrence of polypoid process. To evaluate the effect of telmisartan to indicators CAP, PWV and cognitive function in workers of locomotive crews with hypertension of stage I. Materials and methods. In 36 workers of locomotive crews with hypertension stage I was evaluated the effect of 12 weeks treatment with telmisartan on blood pressure indicators, CAP and PWV, as well as cognitive function. Results.During therapy with telmisartan was a statistically significant reduction in BP, CAP, PWV. There was no effect to the cognitive functions. Conclusion. Telmisartan may be regarded as an effective and safe drug for the treatment of hypertension in workers of «dangerous professions». Materials and methods. The study involved 320 women living in the territory of city Krasnoyarsk. Methods: anthropometry, the calculation of indices, statistical processing. Results. The study found that among the women of the city are dominated representatives of the mesomorphic and andromorfic body types. In women of all body types was a trend toward decreasing the size of the pelvis. Conclusion. In 50% of cases in women was identified anatomically narrow pelvis. According to the classification forms of the narrow pelvis in women of all body types was prevailed transverse narrowing form. Materials and methods.All objects of the research were conducted of anthropometric and somatotypical examination. Results.At the corpses of men who died from heart disease, significantly more often percentage of people with mesomorphic body type by classifications of Shevkunenko, Chernorutskiy and Rees-Eisenck, andromorphic by Tanner, with the shape of the abdomen, extended down by Zhukov, overweight and high-density body. Conclusion.The obtained results allow to study further investigation to identify external biomarkers depending on the specific nosological unit, part of the heart, the depth and degree of the lesion. To identify ethnic features kefalometric parameters and head shape of the contemporary women of Eastern Siberia compared to the similar data of the European part of Russia. Materials and methods. The study involved 239 women – Europeoids and 406 women - Mongoloids. Was conducted kefalometria followed by determination of the cephalic index and head shape. Results. Larger head size is typical for the Mongoloid population compared to Europeoids. In the temporal aspect (300 years) the decrease of cephalic index in Europids is typical. Conclusion. Our results support the view of T.I. Alexeeva about common for the entire Russian population anthropological type on cephalic index – mezokefalia. To compare the level of allergic morbidity in children with the ecological conditions where they live. Materials and methods. Was analyzed the environmental situation and the dynamics of indicators of allergic disease in children of 0-14 years old and 15-17 years old in different regions of the Russian Federation according to official statistics for the period 2001-2011. Results. The analysis of air shows a strong tendency to decrease the number of samples exceeding the hygienic standards as in Russia in whole, so in all federal districts. However, in 35 territories was marked air pollution at 5 maximum allowable concentrations and more. Sanitary condition of water bodies remains unsatisfactory. It was revealed the growth of allergic diseases in children aged 0-14 years old and 15-17 years old. Conclusion. The interrelation between the prevalence of asthma, allergic rhinitis, atopic dermatitis with environmental features (level of pollution of air and water) of the subjects of the Russian Federation was established. To assess the significance of putative risk factors that contributed to the lactostasis and lactation mastitis in nursing mothers. Materials and methods. The study included 1,005 mothers of healthy term breastfed infants. Results. Analysis of the data showed that lactostasis and lactation mastitis does not affect to the development of children. Predicting the probability of these diseases in clinical practice is unreasonable due to their low sensitivity and specificity. In fact, the only real predictor is the presence of lactation in women. Conclusion. Considering heterogeneity of the data, requires further study of the problem with the maximum number of different factors. To study etiological structure and features of the clinical course of professional bronchitis in Krasnoyarsk Region. Materials and methods. Was conducted clinical-statistical analysis of 172 case histories of patients with professional bronchitis. Results. Professional bronchitis have dust (24.4%) and toxic-dust (75.6%) etiology. Patients with toxic-dust bronchitis significantly more often (p <0,001) was severe variant of the disease. Professional bronchitis most often combined with cardio - vascular pathology. In 72.7% of cases the disease has a steady course. Conclusion. Toxic-dust bronchitis are diagnosed 3 times more often than the dust, and are characterized by the more aggressive course. To study the influence of manual therapy on neurological and hemodynamic manifestations in patients with chronic cerebral ischemia. Materials and methods. There were 100 patients with chronic cerebral ischemia. Patients were divided into two groups depending on the treatment, which efficacy was assessed after a certain time periods: on the 20th day, after 3 and 6 months after treatment. Results. Vertebrogenic influence on the vegetative innervation of the brachiocephalic vessels had primarily angiodystonic character, and, as a consequence, led to hemodynamic and neurologic manifestations. It was found that the use of soft-tissue techniques course of manual therapy with standard neurometabolic therapy gives the most effective outcome. Conclusion. The use of soft-tissue techniques of manual therapy leads to better blood flow of the brain and reduces the neurological manifestations. To study the effectiveness of allergen-specific immunotherapy (ASIT) in patients with atopic bronchial asthma (ABA). Materials and methods. The study involved 174 patients with ABA, both sexes receiving ASIT. All the patients were underwent the study of external respiration function, scarification samples, allergen-specific immunotherapy, the statistical analysis of the results of research was carried out with the help of software package Statistica-6.0 for Windows. Results. The results of the 12 month follow-up demonstrated the efficacy of ASIT in patients with ABA of mild and moderate degree. Against the background of immunotherapy there is not only a decrease in the frequency of symptoms and exacerbations of the disease, but also the opportunity to reduce doses of inhaled corticosteroids. Conclusion. ASIT – a highly effective method of treatment in patients with atopic asthma, positively effecst on the nature of antiinflammatory therapy and reduces the dose of basic drug. Evaluating the effectiveness of vertical transmission of HIV in Krasnoyarsk Region. Materials and methods. All data were obtained by continuous method. Descriptive statistics are presented by absolute values, extensive and intensive coefficients. Results. Increasing in 2.5 times in Krasnoyarsk region over the past 10 years the number of children born by HIV-positive women. Reduction in 6.3 times the number of HIV-infected pregnant women who did not have chemoprophylaxis of HIV transmission from mother to child. Reduction of perinatal HIV transmission to 5.3%. Conclusion. Increasing of coverage of 3-steps chemoprophylaxis of pregnant women leads to decreasing the number of children with perinatal HIV infection. To study the frequency, spectrum and structure of primary drug resistance in patients with pulmonary tuberculosis, first identified in terms of the penitentiary system of the Krasnoyarsk Region and were hospitalized in a medical facility of FSIN Russia. Materials and methods. Drug resistance MBT strains detected in 102 patients with standard indirect method of absolute concentrations. Results. Primary drug resistance occurred in 70.6%; was dominated multiple DR (48.1%). Conclusion. Requires accelerated methods for determining the LU MBT, allows to have adequate therapy in a shorter time. To evaluate the effect of combined drug meteospazmil on the severity of the clinical manifestations of different variants of irritable bowel syndrome. Materials and methods. The study included 68 patients with different types of irritable bowel syndrome (IBS). Treatment efficacy was assessed using a visual analog scale intensity of abdominal pain – VAS, verbal and non-verbal rating scale pain – RSP. Results. By the end of the first week of therapy was significantly reduced the intensity of abdominal pain, bloating severity, by the end of the treatment of pain was almost completely arrested the vast majority of patients. Conclusion. Meteospazmil drug has proven efficacy in relieving abdominal pain, normalization of stool frequency and consistency in patients with different types of IBS. The authors presented two cases of simultaneous bilateral choroidal detachment. The possible reasons for its development are given as sharp decrease of blood pressure and a mixed form of endocrine ophthalmopathy in subcompensation stage. In the first case the clinical manifestation of choroidal detachment was stopped independently within five days, in the second case in addition to traditional drug therapy it was required to overlay binocular pressure bandage and bed rest.
