DOI 10.20333/25000136-2023-3-5-11 Authors Annotation Traditional surgical aortic valve replacement is the «gold standard» for the treatment of patients with aortic stenosis. Transcatheter aortic valve implantation is an alternative treatment for these patients. The article deals with the issue of conduction disorders in patients with aortic valve defects. Due to the wide distribution of this type of lesion of the valvular apparatus of the heart and the high risk of complications and mortality, this pathology is an urgent problem of modern cardiology. The data of a retrospective assessment of the frequency and nature of conduction disorders, both in surgical aortic valve replacement and in transcatheter aortic valve implantation, are presented. The most common complication after surgery was arrhythmia in the form of stage II-III atrioventricular block, including those requiring implantation of a permanent pacemaker. Also, conduction disorders depended on the type of prosthesis used, and bundle branch blockades (left anterior fascicular block, RBBB, LBBB) were an independent predictor of mortality. 18.. DOI 10.20333/25000136-2023-3-12-18 Authors Annotation A literature review was conducted on the issues of peritonitis severity prediction. The review was conducted using the following databases: PubMed, Scopus for the period from 2009 to 2021 using the following key words: common peritonitis, abdominal sanitation, prognostic scale, laparoscopy, laparotomy. Early prognostic assessment of the severity of advanced peritonitis largely determines the surgical tactics of treatment, and also makes it possible to predict the course and outcome of the disease. The article discusses the most common scales for assessing the severity of peritonitis, analyses their advantages and disadvantages, and the possibilities of application. It describes its own scale, which, based on indicators of oxidative stress and markers of inflammation, allows us to assess the need for laparoscopic sanitation of the abdominal cavity with advanced peritonitis and predict its efficacy. Further research is needed to assess the information content of prognostic scales for assessing the severity of peritonitis with various methods of abdominal cavity sanitation. DOI 10.20333/25000136-2023-3-19-26 Authors Annotation Ankylosing spondylitis (AS) or Bekhterev’s disease (BD) is an autoimmune rheumatic disease affecting spinal column joints, sacroiliac joint and adjacent soft tissues, resulting in myalgia, tendinitis and enthesopathy. With progression and in case of treatment failure, the disease may lead to fibrosis and calcification, resulting in loss of flexibility and bamboo-like spinal fusion. The main clinical manifestations include back pain and progressive decrease in mobility of spinal joints as well as inflammation of the hip and shoulder joints, peripheral joints, and fingers / toes. In addition, there are extra-articular manifestations such as acute anterior uveitis and inflammatory bowel disease (IBD). Like any autoimmune disease, it develops as a result of complex relationships between the genetic background and environmental factors. Although significant progress has been made in the study of BD in recent decades, the aetiology remains completely unknown. To the date, research has identified several factors closely related to the onset of AS, including genetics, immune responses, microbial infections, and endocrine system influence. One of the most discussed diseases in recent years has not spared pregnant women either. Unfortunately, the effects of the virus on the body undergoing known physiological changes associated with pregnancy often do not pass without consequences for both the female and the foetus. The impact of vaccination on foetal health and development despite receiving data from many researchers on its safety remains a debatable issue given the presence of systemic immune responses. The review presents literature data on the impact of coronavirus on pregnancy and birth outcomes, the results of foreign studies on the consequences of COVID-19 vaccination for pregnant women and its relationship with lactation and breastfeeding. To evaluate antimicrobial activity of three variants of gels in vitro, as well as to study their efficacy in relation to prevention of implant-associated osteomyelitis in an experiment on rats. Material and methods. The work involved studying three variants of antibacterial gels based on medium molecular weight polyvinylpyrrolidone (PVP) containing antibacterial products. The experiment was carried out in vitro. At the first stage, the sensitivity of type strain S.aureus АТСС 29213 to the active substance of gel specimens was investigated. The research was performed in the vivarium of FSBI «R.R. Vreden NMRC» on 23 sexually mature female white outbred rats. All animals were implanted with wires into the proximal part of the femur in different groups. For the control animals (n=5), the infected wire was not treated with gel. In experimental groups I, II and III (n=9 for each), the wire was treated with the studied antimicrobial gel. Euthanasia of the animals was performed on day 21 after the procedure; the microbiological and further morphological study was performed within this period of time. Results. The presence of antibacterial activity in relation of the type strain S.aureus АТСС 29213 was registered in all three specimens of the gel. Application of gels1 and 3 made it possible to achieve eradication of Staphylococcus in two thirds of the cases. The most pronounced antibacterial effect was found while using gel 2. Morphological studies demonstrated a histological picture of acute osteomyelitis in control animals (sequesters and segmented neutrophils in fibrin accumulations 17.9±9.4 per field). Average quantity of neutrophils in groups I, II and III was comparable but significantly lower (p<0.001) than in the control group: 3.4±2.7, 3.7±4.3 and 3.0±1.6 cells per field. Conclusion. The use of all the three variants of experimental gels did not reveal substantial differences in their antibacterial activity. Local antibacterial therapy was effective and made it possible to prevent development of acute osteomyelitis after implantation of an infected metal construction in all experimental animals. However, in occasional observations, areas with neutrophilic infiltration in tissue around the bone marrow cavity were visualised. The study has shown that development of polymeric antibacterial gels is a promising field for research in development of preventive means and for treatment of implantassociated infection in orthopaedics and traumatology. To evaluate the results of musculoskeletal lesions screening in psoriasis and clinical characteristics of patients with psoriatic arthritis in Krasnoyarsk region. Material and methods. The study for the period from 12.03.2022 to 01.12.2022 included 336 patients with PsO (42.0 [32.0/56.0] years), including 178 (53%) men (40.5 [31.0/54.0] years) and 158 (47%) women (43.5 [34.0/59.0] years, p=0.039). Most of the subjects were undergoing inpatient treatment: 140 (42.4%) in day care and 96 (29.1%) in round-the-clock hospital, the remaining 94 patients (28.5%) were observed on an outpatient basis. The vast majority of patients - 303 (91.8%) had a plaque form of psoriasis. All patients were assessed for the severity of skin lesions (Psoriasis Area and Severity Index – PASI), quality of life (The Dermatology Life Quality Index – DLQI); screening of musculoskeletal system lesions included questionnaires mPEST (modified Psoriasis Epidemiology Screening Tool) l, BASDAI (Bath Ankylosing Spondylitis Disease Activity Index) and inflammatory pain in back (A.Calin et al., M.Rudwaleit et al., ASAS). The study was conducted in compliance with the requirements of good clinical practice; all patients signed an informed consent. Results. We have not revealed statistically significant gender differences in the severity of skin lesions and assessment of the quality of life of patients with PsO. Thus, 168 patients (50.0%) had severe skin lesions (PASI ≥ 20 points), p=0.537, 91 patients (27.1%) had moderate severity (PASI 10-19 points), p=0.513, 77 (22.9%) had mild skin lesions (PASI 10 points), p=0.203. More than half of all patients – 145 (52.2%) – had 11-20 points (very strong influence on quality of life) and 21-30 points (extremely strong influence) on DLQI (p=0.083). At the same time, a significant prevalence of comorbid background was revealed in women suffering from PsO. A total of 183 patients (54.5%) had concomitant pathology, 99 (62.7%) in women, 84 (42.7%) in men, p=0.004. Women, as compared with men, more often suffered from heart rhythm disorders (7.0% and 0.0%, p<0.001), liver and pancreatic diseases (13.9% and 4.5%, p=0.002), diabetes mellitus (8.2% and 2.2%, p=0.013). Every third woman had two or more concomitant diseases – 61 (38.6%) and every fourth man – 44 (24.7%), p=0.006. Probable PsA (mPEST ≥ 3 points) was diagnosed in one third of patients – 117 (34.8%). The association of mPEST ≥3 with age (46.0 [37.0;57.0] vs. 40.0 [30.0;56.0] years, p=0.012), longer disease experience (18.0 [7.0;30.0] vs. 12.0 [5.0;23.0] years, p=0.048), with a more pronounced effect on quality of life was revealed (DLQI 13.5 [9.0;19.0] vs. 10.0 [6.0;15.0] points, p=0.001), severity of skin lesions (PASI 30.7 [26.7; 37.3] vs. 25.6 [22.0; 31.0] points, p= 0.00) in almost every other patient (49.6%). A total of 143 (42.6%) patients reported various types of spinal pain. At the same time, 85 out of 117 patients (72.6%) had this symptom in patients with mPEST ≥3, whereas only 58 out of 219 (26.5%) had this symptom in patients with mPEST less than 3, p<0.001. However, only 33 out of 143 met ASAS criteria for inflammatory back pain (23.1%) of the patient, while more than half of the patients – 73 out of 143 (51.0%) – met at least one of the four criteria used. Conclusion. Timely diagnosis of PsA is an urgent and unsolved healthcare task. Interdisciplinary interaction between a dermatologist and a rheumatologist and conducting an in-depth examination based on the results of mPEST screening is necessary for virtually every third patient (34.8%), and clarifying the nature of the axial lesion is necessary for every fifth patient (21.7%). To study comorbid associations of recurrent cephalalgia in adolescents from the rural population of Khakassia. Material and methods. The object of the study is random samples of adolescents aged 12-18 years from 6 schools in 3 rural districts of Khakassia amounting to 612 subjects, 254 (41.5%) male and 358 (58.5%) females, median (Me) of the age of the examined totalled 15.0 [14.0; 16.0] years. Recurrent cephalalgia (RC), abdominal and back pain (dorsalgia), and asthenic syndrome (AS) were verified using a screening questionnaire. Online behaviour was assessed according to the Chen scale (CIAS) and the level of anxiety was assessed using the test by C.D. Spielberger and Yu.L. Khanin. Compared indicators in groups by sex and age (12-14 and 15-18 years). The data were processed using the Statistica 12 program. Results. Rare cephalalgia was registered in 32.3% of the total sample, frequent – in 5.7%. The overall incidence of cephalalgia (rare and frequent) is higher among females (49.2% and 22.4%, p<0.0001) and in older adolescents (46.8% and 23.1%, p<0.0001). ). The age-sex structure included a high prevalence of cephalalgia (both rare and frequent) in girls and in older adolescents. АIU was more often associated with the absence of cephalalgia, NIU and PIU were associated with rare cephalalgia, and PIU was associated with frequent cephalalgia. The most significant associations of frequent cephalalgia were with AS (OR=14.37, CI 5.90-35.09), frequent abdominal pain (OR=10.58, CI 4.53-24.74), back pain (OR= 7.43, CI 3.38-16.37), PIU (OR=4.26, CI 1.57-11.58), NIU (OR=2.21, CI 1.06-4.63), DIU (OR=4.17, CI 1.82-9.58) and high personal anxiety (OR=4.41, CI 2.16-9.01). Rare cephalalgia was significantly more often associated with AS (OR=4.92, CI 2.50-9.68), frequent back pain (OR=4.83, CI 2.96-7.90), high reactive anxiety (OR =2.59, CI 1.73-3.89), Internet game addiction (OR=11.24, CI 1.34-94.59) and associated back pain (OR=11.27, CI 1.31-97.58). Conclusion. Exemplified by the surveyed sample, the prevalence and structure of recurrent cephalalgia in adolescents of the rural population of Khakassia, their age and sex differences and significant associations with a number of indicators have been established. To study the chemiluminescent activity of neutrophilic granulocytes and monocytes in blood of children with Helicobacter pyloriassociated erosive and ulcerative lesions in the stomach and duodenum. Material and methods. The objects of the study were neutrophilic granulocytes and blood monocytes isolated from 46 children with H. pylori-associated erosive and ulcerative lesions in the stomach and duodenum at the age of 11 to 18 years and from 55 apparently healthy individuals. The diagnosis was established using clinical and laboratory analysis. Determination of oxygen-dependent phagocytosis of blood monocytes and neutrophilic granulocytes was carried out through the chemiluminescent method. Results. The study of oxygen-dependent phagocytosis of blood neutrophilic granulocytes in the group of patients showed a significant decrease in the timeto-peak and an increase in the intensity and area under the curve of the luminol-dependent spontaneous and zymosan-induced response compared to the control range. The activation index in the group of patients decreased significantly. The intensity of blood monocytes in the group of patients is reduced compared to the control group. A comparative study of respiratory activity showed an increase in intensity in the luminol-dependent spontaneous and zymosan-induced process in neutrophilic granulocytes, while the area under the curve is reduced as related to blood monocytes. The study of the lucigenindependent process showed a significant increase in the activity of the general population of neutrophilic granulocytes (intensity and area under the curve) both during the spontaneous reaction and in the zymosan-induced process relative to blood monocytes. At the same time, the activation index in blood neutrophils is increased as related to the monocytes. Conclusion. Thus, there is an intensification of the “respiratory burst” in the total fraction of neutrophilic granulocytes in the blood isolated from patients with H. pylori-associated erosive and ulcerative lesions in the stomach and duodenum, in a spontaneous process and during induction with zymosan as compared to the controls. The activity of blood monocytes is significantly reduced. The results of the comparative assessment characterise the insufficiency of the cytotoxic activity of monocytes in comparison with neutrophils in patients with H. pylori-associated erosive and ulcerative lesions in the stomach and duodenum. To study serum levels of RANKL and OPG in adolescent girls with undifferentiated connective tissue dysplasia (UCTD) with menstrual disorders (MD). Material and methods. A survey of 216 adolescent girls aged 11-17 years (average age - 14.0±0.15 years) was carried out. All subjects were divided into 2 groups: 176 patients with established MD and UCTD (main group) and 40 healthy adolescent girls (control group). In blood serum samples isolated in the morning on an empty stomach, the levels of receptor activator of nuclear factor-κB ligand (RANKL) and osteoprotegerin (OPG) were determined. The concentration of cytokines was determined by enzyme immunoassay. Results. Adolescent girls with UCTD and MD showed a statistically significant increase in RANKL concentrations (p<0.05) with a trend towards an increase in OPG values (p<0.1). In addition, adolescent girls with UCTD and MD were significantly more likely to have RANKL levels over 3.0 pg/mL (56.8% versus 35.0% in the control group; p=0.020) and OPG over 110 pg/mL (19.3% versus 2.5% in the control group, p=0.018). Conclusion. The established increase in the production of RANKL and OPG cytokines may reflect the peculiarities of the regulation of bone tissue metabolism in adolescent girls with MD and UCTD and can be used to develop effective individualized schemes for treatment and prevention. DOI 10.20333/25000136-2023-3-71-77 Authors Annotation The development of venous thromboembolism in patients with brain malignant glioma is one of the reasons of significant decrease in their life expectancy. That is why detection of susceptibility to thrombosis is a relevant problem in modern neuro-oncology. The aim of the research. The purpose of this study is to perform comparative analysis of the coagulation parameters of blood in patients with brain tumour pathology in the preoperative period with subsequent study of the prospects of using the obtained results to increase their life expectancy. Material and methods. The study included 48 brain tumour patients: malignant tumour was diagnosed in 25 (52.1%) patients (group 1); benign tumour was detected in 23 (47.9%) patients (group 2). In the preoperative period, hypercoagulation was established according to the results of the “Thrombodynamics” test and based on the parameters of the standard coagulogram. Results. Venous thromboembolism was only diagnosed in 4 (8.3%) patients. According to the “Thrombodynamics” test and coagulogram, hypercoagulation was detected significantly more often: in 37 (77.1%) and 19 (39.6%) patients, respectively. The “Thrombodynamics” test results indicated that the increase in induced clot formation rate in group 1 was significantly higher than in group 2 (55 [33; 66] μm/min and 40 [32; 52] μm/min; p=0.043). Also, in group 1, according to “Thrombodynamics” test, a tendency to shorten the time of spontaneous clot formation was established. Coagulogram parameters also attested to the decrease in activated partial thromboplastin time in group 1 (p=0.003). Conclusion. In the preoperative period, hypercoagulation is detected much more often than venous thromboembolism in patients with brain tumour. Patients with malignant tumour have a higher tendency to thrombosis. The results of “Thrombodynamics” test may become key in development of schemes for preventive procedures to reduce the degree of hypoxia in the brain tumour zone and for increasing the efficacy of adjuvant radiation therapy. To study and compare metabolic disorders in alloxan diabetes, as well as to evaluate the feasibility of using metformin and insulin in laboratory animals against the background of experimental diabetes. Material and methods. The study was carried out on 30 female Wistar rats. Laboratory animals were divided into 4 groups (the control group, the group with simulated diabetes, the group with experimental diabetes injected with metformin, and the group with experimental diabetes injected with insulin). For the study, blood was taken from the retro-orbital sinus of the eye. Biochemical parameters were studied: glucose, triglycerides, total cholesterol. Electrophoretic profiles of lipoproteins: VLDL, LDL, HDL were analysed. Results. The results showed that alloxan causes a decrease in the body weight of experimental animals. This decrease is more pronounced in animals treated with insulin. The evaluation of glucose levels revealed that the decrease in glucose levels was 15% in animals treated with metformin for 2 months and 56% in animals treated with insulin. Taking metformin caused a significant decrease in hypertriglyceridemia after one month of treatment with the preservation of indicators and in the second month. Taking insulin did not cause improvement in hypertriglyceridemia after one month of treatment and the result was achieved only after the second month of treatment. The introduction of insulin and metformin led to a decrease in cholesterol levels after two months of treatment, and when using metformin, the result was significant during the first month of treatment. While analysing electrophoretic profiles of lipoproteins, metformin treatment showed a decrease in LDL area. The areas of VLDL and HDL showed little change. In the case of insulin treatment, an improvement in the electrophoretic profile was recorded: measurement of the areas showed their decrease for VLDL, LDL, and HDL. Conclusion. Alloxan leads to the development of diabetes, which is characterised by hyperglycaemia, hypercholesterolemia and hypertriglyceridemia. The introduction of insulin and metformin led to a regression of metabolic disorders: anti-hyperglycaemic effect, a decrease in cholesterol, triglycerides, LDL, LDL and HDL. When comparing the characteristics of insulin and metformin, it was found that insulin has a stronger hypoglycaemic effect. Metformin significantly affects the level of triglycerides and cholesterol and acts after a shorter period of time. To study the possibility of complex analysis of some blood serum chemokines as diagnostic biomarkers of the severity of COVID-19 in preschool children. Material and methods. Multiplex analysis was conducted for quantification of chemokines CXCL1 (GROα), CXCL5 (ENA-78), CXCL9 (MIG), CXCL11 (I-TAC), CCL3 (MIP-1α), CCL4 (MIP-1β), CCL5 (RANTES), CCL11 (Eotaxin), CCL17 (TARC), CCL20 (MIP-3α) in blood serum using flow cytometry in children with COVID-19 (main group, n=21; divided into asymptomatic (n=7), mild (n=7) and moderate (n=7) forms in comparison with healthy children (control group, n=25). Results. The levels of chemokines CXCL1 (GROα), CXCL5 (ENA-78), CCL3 (MIP-1α), CCL4 (MIP-1β), CCL5 (RANTES), CCL11 (Eotaxin), CCL17 (TARC), CCL20 (MIP-3α) were higher in preschool children with COVID-19 than in healthy ones. The blood level of CXCL9 (MIG) and CXCL11 (I-TAC) did not differ between two groups. There were no statistically significant differences between the levels of separate chemokines and the severity of COVID-19. However, it has been shown that comprehensive assessment of CXCL1 (GROα), CXCL5 (ENA-78), CXCL9 (MIG), CXCL11 (I-TAC), CCL3 (MIP-1α), CCL4 (MIP-1β), CCL5 (RANTES), CCL11 (Eotaxin), CCL17 (TARC), CCL20 (MIP-3α) serves as an indicator of early diagnosis of the severity of COVID-19 in children. Conclusion. Analysis of immune biomarkers can help differentiate the severity of COVID-19 in preschool children when they are admitted to a hospital. A detailed assessment of the chemokine profile provides additional information about the severity of COVID-19 in children that can help to determine further patient management. To carry out a comparative study of the wound process on a purulent wound model using a developed multicomponent wound dressing. Material and methods. The material for the study was a wound dressing in the form of a film developed by the authors and patented (patent for invention 2603490), which was used for treatment of a purulent wound model in the experimental group. In the control group of animals, the treatment was carried out with the official dioxomethyltetrahydropyrimidine ointment with chloramphenicol. Preliminarily, the official drug and the developed wound dressing were studied for the spectrum of antimicrobial and fungicidal activity, and the local anaesthetic activity of the developed dressing was studied in comparison with lidocaine ointment. Subsequently, an experiment was performed on 72 laboratory rats, divided into two equal groups, to which a purulent wound was modelled. The course of the wound process was assessed by the external state of the wound, the percentage of decrease in the of wound area, the rate of healing, the level of contamination of wounds were calculated, and cellular elements were counted on histological preparations of wound sections (percentage of fibroblasts, macrophages, lymphocytes and granulocytes), the cellular index was calculated and the level of pH of the wound surface was determined. Results. In the experimental group, the reduction in the of wound area occurred faster compared to the control group, and by the fifteenth day it decreased by more than 98%, and by 88.3% in the control group, with the maximum healing rate on the first five days of the experiment. In the experimental group, the decrease in the contamination of the wound was significantly faster, and the pH values of the wound surface were restored. Morphometric parameters indicated a rapid increase in fibroblasts and a decrease in the level of granulocytes in the experimental group compared to the control group, the differences were statistically significant. The predominance of proliferative processes over inflammatory ones occurred significantly earlier in the experimental group (the cellular index was more than 1 as early as on the third day of treatment). Conclusion. A comprehensive study of the features of the wound process has shown that the use of the developed wound dressing influenced the healing process of a purulent wound favourably. DOI 10.20333/25000136-2023-3-100-106 Authors Annotation Bacterial cerebral aneurysms are a formidable complication of infective endocarditis, their rupture is accompanied by high mortality. Treatment of this pathology in the acute period of haemorrhage is a difficult task. Currently, there are no standards and clinical guidelines for the management of such patients, and modern publications are presented only by the analysis of individual series of observations. The article presents the experience of treating 10 patients with bacterial aneurysms in the acute and sub-acute periods of infective endocarditis. A total of 6 patients underwent microsurgical, and 1 was subject to X-ray endovascular exclusion of the aneurysm. In all cases, the aneurysm was excluded together with the carrier vessel. None of the observations after the operation showed the appearance (increase) of neurological deficit or formation of ischemic zones in the surgical area. In this regard, destructive interventions in the distal localisation of bacterial aneurysms are a safe method of treatment in most cases. The study of the causes of bleeding after duodenum-sparing resections, and the development of methods for surgical correction. Material and methods. A total of 164 patients with chronic pancreatitis were examined. The mean age of the patients amounted to 47.5 years. There were 122 male (74.4%) and 42 female (25.6%) patients. Medical history of acute pancreatitis was in 47 patients. Frey’s procedure was performed on 157 (95.7%) patients and Beger procedure was performed on 7 (4.3%) patients. All patients were examined using clinical tests, computed tomography with bolus contrast and EUS in some cases. Results. Bleeding into the lumen of the pancreatointestinal anastomosis occurred in 14 (8.5%) patients. The lethal outcome was registered for 7 patients (4%). In 5 (3%) patients, emergency catheterisation and embolisation of the gastroduodenal artery was performed. In 4 cases, stable haemostasis was achieved. Relaparotomy, removal of the anterior lip of the pancreatointestinal anastomosis, revision of the anastomotic chamber, lavage and haemostasis were performed when it was not possible to carry out angiography. After embolisation, one patient had signs of acute pancreatitis. In two observations, there was a failure of the pancreatointestinal anastomosis. One patient had a recurrent bleeding after stitching. A bleeding prevention technology was proposed, including the use of an original fibrin composite in the anastomotic chamber. Conclusion. Bleeding into the lumen of the pancreatointestinal anastomosis after Frey’s procedure is characterised by a decrease in blood pressure and the appearance of melena. The most accurate diagnosis is carried out by angiography after catheterisation of the gastroduodenal artery. Subsequent embolisation may be effective. In a severe condition of the patient, an emergency relaparotomy, removal of the anterior lip of the pancreatointestinal anastomosis and haemostasis by stitching the parenchyma with subsequent restoration of the integrity of the anastomosis are indicated. The use of the original fibrin composite reduces the risk of bleeding. DOI 10.20333/25000136-2023-3-112-116 Authors Annotation Pregnancy after assisted reproductive technology (ART) is associated with high obstetric and perinatal risks, including placenta accreta. The frequency of placenta accreta increases both with the use of ART in general and with the protocols of embryo cryopreservation and hormone replacement therapy in particular. The article reports on two cases of a favourable outcome of a term pregnancy that was followed ART and complicated by a partial placenta accreta. Both patients underwent planned caesarean section. Intraoperatively, partial placenta accreta was found, therefore, additional resection and reconstruction of the uterine wall was performed. There were blood loss 1100.0 ml and 1450.0 ml during surgeries. The postoperative period was uneventful. The level of chorionic gonadotropin in the blood was measured 2 and 3 months after surgery and was <2 mIU/ml.
