It was first described in 2019, developed into an epidemic in January 2020 and has spread the global to the present COVID-19 pandemic. Specialized pro-resolving mediators (SPMs) may play a new role in the management of this lung disease because SPM actively stimulate the resolution of infectious infl ammation and are organ protective in animal disease models. Many tissues have been suitable targets for treating inflammation with SPMs or their active precursors 18-HEPE, 17-HDHA and the 14-HDHA, in order to elicit dynamic resolution of inflammation. Despite the fact that mammary gland is a surface-located organ easily accessible for examination, the echographic images of malignant tumors can have many masks, especially when tumors are of small sizes. The present article is a literature review highlighting modern ultrasound examination methods that can expand the protocol for comprehensive diagnosis of mammary gland cancer. Current works are aimed at studying microvascular bed, including the use of echo-contrast drugs. DOI 10.20333/2500136-2020-4-29-35 Authors Annotation The aim of the research is the assessment of obesity frequency according to various criteria, depending on socio-economic factors. Material and methods. 1,600 of 35-70-year olds were examined. Th e average age of the respondents was 54.3 ± 9.88 years. Obesity was assessed by 5 criteria: body mass index (BMI), waist circumference (WC), waist / hips index (WI / HI), visceral fat rating (VFR), visceral obesity index (VOI). A survey was conducted for all the study participants in order to get information about the level of education, professional affi liation and living conditions. Th e level of household income per month in rubles was also specifi ed. Statistical data processing was carried out by Statistica 6.0 application soft ware. Results. Th ere was no statistically signifi cant diff erence between the rural and urban male residents in the incidence of obesity rate (p = 0.474), however, the obesity rate among rural females was statistically signifi cantly higher than among urban females (p = 0.0000). By means of regression analysis, it was found that living in the city was associated with a decrease in BMI by 1.74 kg / m2 (p <0.0000); by 1.1 units of visceral fat (p <0.0000); by 6.6 cm in waist circumference (p <0.0000). Higher education was associated with a decrease in BMI by 1.19 kg / m2 (p = 0.002); WC / HC by 0.02 (p = 0.000003); WC by 0.11 cm (p = 0.0004); the part of internal fat by 0.67 units (p = 0.005); VOI by 0.30 (p = 0.016). High level of income is associated with an increase in BMI by 0.75 kg / m2 (p = 0.026), the part of visceral fat by 0.63 units. (p = 0.002). Conclusion. Obesity prevalence is higher in rural areas compared to urban areas due to females according to all the studied criteria, except for visceral fat rating (according to this criterion, obesity prevalence in males is higher than in females). Females with higher education have the lowest obesity prevalence according to all the studied criteria. Female graduates are less likely to suffer from obesity, determined by the criteria of BMI, WI / HI and VFR. Such factors as urban living, being a female and aging are negatively associated with indicators characterizing the presence of obesity. Higher education was associated with a decrease in the above-mentioned indicators. High income was associated only with an increase in BMI and the proportion of internal fat. Dr.Med.Sci.; Scientific Research Institute of Medical Problems of the North; ТАК34@yandex.ru Annotation The aim of the research is to study clinical and epidemiological features of chronic hypertrophic rhinitis associated with mycoplasma infection in adults. Material and methods. Verifi cation of Mycoplasma pneumoniae (M. pneumoniae) was carried out in smears of mucous membrane taken from the middle nasal passage and biopsies by polymerase chain reaction in 86 people with chronic hypertrophic rhinitis (CHR) who received hospital treatment at Research Institute of Medical Problems of the North in Krasnoyarsk City. All the patients underwent clinical and anamnestic examination. Comparative analysis of the course of CHR was performed in two groups of patients, depending on identifi cation of M. pneumoniae, based on clinical and anamnestic examination. The average age of the examined groups was 33.1–1.1 g. Moreover, there was a study of hydrogen ion index concentration (pH-metry) in nasal secretion by calorimetric method. Results. The studies have shown high incidence of mycoplasma infection in adults hospitalized for acute maxillary sinusitis. Th ere are clinical and anamnestic features of mycoplasma infection manifestation in patients with CHR, characterized by longer course, additional complaints, frequent exacerbations, severe hypertrophy of mucous membrane not only of the nasal cavity, but also of paranasal sinuses. Characteristic mucociliary clearance of nasal mucosa in patients with CHR and influence of M. pneumoniae infection on its level were revealed. Conclusion. The course of the disease in patients with CHR in case of mycoplasma infection did not have characteristic specifi c clinical manifestations, however, some features were observed. Voyno-Yasenetsky Krasnoyarsk State Medical University; aasavchenko@yandex.ru Annotation The aim of the research is the assessment of schoolchildren health status during a 10-year observation (during the periods of high risk of stressful situations development) from view point of analyzing the nature and frequency of respiratory diseases detection and studying some metabolic parameters of leucocytes. Material and methods. The paper presents the results of health status study of 437 schoolchildren at diff erent stages of school education (before coming to school, at the end of the fi rst grade, during the transition to subject education and at the end of school education) and depending on the age when school education has started. Results. The number of children without diseases of upper respiratory tract is decreased at the indicated stages of observation, while acute and chronic infections (tonsillitis, otitis media, pharyngitis) prevail in the structure of the pathology. In children with frequent respiratory diseases, the increase in the activity of the studied parameters of enzymatic status of leukocytes (SDH, CK) was determined, indicating the increase in functional activity of immunocompetent cells (mainly by the end of school). Conclusion. Material and methods. A prospective cohort clinical study was conducted. 50 children, aged 2- 5 years with a positive index of predisposition to bronchial asthma (API) were examined. Among them, 30 children (the main group), along with the treatment of acute respiratory infections and bronchial obstruction, took montelukast at a dose of 4 mg per day at night, during three months; patients of the comparison group (n = 20) received symptomatic therapy for acute respiratory infection, including bronchodilator. Results. Th e study has showed that the use of the medication for three months is highly eff ective (89.7%) and provides a reduction in the severity of clinical manifestations of respiratory infection, rhinorrhea, intensity of day and night cough, and a decrease in the occurrence of episodes of bronchial obstruction, in particular. Conclusion. Montelukast is an eff ective and convenient medication for the prevention of recurrent bronchial obstruction in preschool children. Voyno-Yasenetsky Krasnoyarsk State Medical University; narkevichart@gmail.com Annotation The aim of the research is to study the results of caverns (cavernotomy) open treatment method in case of fi bro-cavernous tuberculosis of respiratory organs. Material and methods. Th ere was a retrospective cohort study of 68 patients with fi bro-cavernous tuberculosis of respiratory system who were treated in Krasnoyarsk Regional Tuberculosis Dispensary № 1 during the period 1998 - 2010 and underwent caverns (cavernotomy) open treatment method. Th e age of patients ranged from 15 to 65 years, with a predominance of the age group over 40 years - 41 patients (60%). Results. A complete clinical eff ect (bacterial excretion has stopped; no destruction of lung tissue; postoperative complications are eliminated; no signifi cant functional damage) was achieved in 52 people (77%). Improvement (bacterial excretion has stopped, but lung tissue destruction is preserved, or elimination of postoperative complications caused functional damage to the patient, or residual cavity still remains) was detected in 11 people (16%). Th ere were no changes (there is still bacterial excretion with stable tuberculosis process and absence of postoperative complications) in 1 person (2%). A fatal outcome (including operative, postoperative and hospital, period up to 90 days, mortality) occurred in 2 people (3%). Conclusion. Th e method of caverns (cavernotomy) open treatment is eff ective in patients with FCT, who, due to prevalence of the process, its activity, low functional parameters, resection surgery is contraindicated or associated with high risk of severe postoperative complications, and collapse surgery in them is ineff ective due to the large sizes of caverns or their localization, as evidenced by achievement of full clinical eff ect in 77% of operated patients, with a mortality rate of 3%, determination of bacterial excretion in 87% of people, clinical cure in the period from 5 to 17 years in 80%, with mortality from tuberculosis in the long term in 15% Keywords: fibrocavernous tuberculosis, surgical treatment, complications, treatment outcomes, treatment eff ectiveness, determination of bacterial excretion,

Material and methods. In the period from March to May 2020, specialists from health monitoring department of Krasnoyarsk Regional Center of Medical Prevention, KSBIHC, conducted a sociological study. Th e study included e-questionnaire, as a result of which 698 respondents from 32 areas of the territory were interviewed. Data processing and all necessary calculations were made in the SPSS soft ware-analytical complex. Results. As a result of the study, it was found that there are some diff erences in the perception of information about coronavirus infection among the age and sex groups. Thus, 82.1% of respondents participating in webinars noted that the information provided by specialists is suffi cient to consider the situation concerning coronavirus infection calmly and feel protected. Conclusion. In general, the eff ectiveness of information infl uence concerning coronavirus infection has been accessed as high. According to the study, the basic principles of work to inform the population of the Krasnoyarsk Territory about coronavirus infection have been formed. DOI 10.20333/2500136-2020-4-71-76 Authors Annotation The aim of the research is to evaluate the eff ectiveness of treatment and chronobiological characteristics of females with overactive bladder (OAB). Material and methods. Th e article describes the problem of lower urinary tract symptoms (LUTS) in females. 40 women aged 36 to 55 years with an overactive bladder were examined. All the patients received basic M-anticholinergic therapy (fesoterodin - 4 mg 1 time / day) during 3 months. Assessment of chronobiological status with Omega-M2 (Dinamika) soft ware and hardware complex (SHC) was evaluated in four visits: at the fi rst visit, in a month, in 2 months, and in 3 months. Also, the patients’ analysis was carried out according to their life quality and psycho-emotional status using the results of urination diaries, assessment of pain intensity during urination on 5-point verbal pain assessment scale, SF-36, HDRS, HARS and IPSS tests. Results. In the course of the study, all the females with overactive bladder who participated in the study had not only symptoms of the lower urinary tract, but also a signifi cant violation of psycho-emotional state and desynchronosis. Conclusion. Th e results of the study indicate that the use of basic cholinolytic therapy stops the symptoms of the lower urinary tract (frequency of mycotomy, imperative urges, nocturia) only by 3 months of treatment. That is why psycho-emotional state of patients is not fully restored and chronobiological rhythms do not normalize, that aff ects life quality of this group of patients. Voyno-Yasenetsky Krasnoyarsk State Medical University; narkevichart@gmail.com Annotation The aim of the research is to compare the eff ectiveness of various methods of hysteromyoma relapse drug prevention in patients of reproductive age aft er myomectomy. Material and methods. During a prospective comparative study, 149 patients of reproductive age were examined in 3, 6, and 12 months aft er myomectomy surgery. All the patients were divided into 3 groups: in the 1st group the patients (n = 30) were prescribed a drug from the group of selective progesterone receptor modulators (SPRM); in the 2nd group (n = 30) a drug from the group of gonadotropin releasing agonists hormone (GnRH) was prescribed; in the 3rd group (n = 30) - combined oral contraceptives (COC) were prescribed. Results. During the dynamic observation by means of ultrasound study, the minimum number of hysteromyoma relapses was detected in the 1st group of patients compared with the 2nd and the 3rd groups. Th e relapse in the fi rst group was observed in 19.6% (n = 10) of patients in 3 months; in 21.6% (n = 11) and 23.5% (n = 12) of patients, respectively in 6 and 12 months. In the 2nd group of the study compared with the 1st group, the increase in hysteromyoma frequency relapses was observed: in 3 months since the start of treatment, hysteromyoma relapses was in 36.7% (n = 18) of patients; in 6 months the relapse rate did not change and amounted 36.7 % (n = 18); in 12 months it was in 40.8% (n = 20) of females. The 3rd group showed the highest number of hysteromyoma relapses: in 3 months the relapse rate was 42.9% (n = 21); in 6 and 12 months the data obtained remained unchanged and also amounted 42.9% (n = 21) cases. According to the results of IHC study, the main prognostic markers in our study were Ki-67 (marker of tumor cells proliferative activity), BCL-2 (marker of apoptosis inhibitor), TGF (transforming growth factor), VEGF (vascular endothelial growth factor), ER (estrogen receptors) and PR (progesterone receptors). Conclusion. It was found that the use of SPRM in the early postoperative period signifi cantly reduces the hysteromyoma relapses frequency, thereby reducing the risk of repeated surgical intervention. (biophysics); msad@icm.krasn.ru Annotation The aim of the research is to evaluate the effectiveness of botulinum toxin in treatment of early upper limb spasticity, taking into account spasticity predictors. Material and methods. Th e study involved 20 patients, 15 patients in the acute period of stroke and 5 patients in the acute TBI period. Neurorehabilitation was carried out in the studied groups. Botulinum toxin type A was additionally used in the experimental group. Muscle tone was assessed using the following functional scales: modified Ashworth scale, modified Tardieu scale, and an assessment of active movements. Th e study evaluated muscle tone in diff erent segments of the upper limb, namely: in shoulder, elbow and wrist joint, as well as the muscle tone of the fi ngers and, separately, of the thumb. In the studied groups, muscle tone was evaluated in three stages. Botulinum toxin was introduced under ultrasound control. IBM SPSS Statistics 23 program was used for data analysis, as well as Microsoft Excel. Statistical processing of the obtained data was performed using Shapiro-Wilk, Student, Mann-Whitney and Kraskal-Wallis criteria, as well as non-linear method of elastic maps. Elastic maps were built using ViDaExpert. Results. Thus, statistically signifi cant diff erences between the studied groups were found for all the indicators, except for those that were removed at the fi rst stage (7-14 days from the onset of the disease and before botulinum therapy in the experimental group) using all three functional scales. Considering that at the first stage in the experimental group, botulino-therapy has not yet been started, the absence of diff erences in muscle tone indicators shows initial relative clinical homogeneity of both groups, that emphasizes the correctness of the study and confi rms the random nature of patients’ distribution into groups. Conclusion. Th e use of botulinum toxin type A in the correction of muscle tone is eff ective when applied precisely at early stages, that allows to recommend active introduction of this method of spasticity treatment in patients during the acute period of focal brain damage into modern medical practice. Initiated in the 1970s, it refers to the concentration and classifi cation of maternity units in order to optimize the maternity hospitals off er on French territory. However, the appropriate optimization is only possible with an understanding of users’ territorial behavior, which is the focus of social and territory justice. Thus we suggest the model which identifies the eff ective demand for care on operational geographical scale, namely, the health territory. The model will be illustrated with the example of maternity hospitals in France. An explanation follows. It leads to systemic thinking. Further principles of systems therapy thinking are introduced: systems (phenomena, people ...) are never isolated, but are always viewed in the context of their environment (this is a young person in a relationship system); systems are in exchange with their environment and adapt to their environment (here we are talking about the adolescent's adaptation to the relationship system); systems can be viewed as constructions for understanding reality and are therefore dependent on the viewer and must not be viewed as “reality”.
