Polymorphism genes associations of vitamin D system with the above mentioned diseases has been found. The article discusses vitamin D metabolism, significant genes that play an important role in vitamin D metabolism and single-nucleotide substitutions in them, associated with low vitamin D concentration in blood and with various diseases. Also there is a review of current recommendations on nutrition and hypovitaminosis medicine treatment in adults. One of the urgent issues of the combined injection and infusion pharmacotherapy is currently an assessment of prescription composition for the pharmacological and pharmaceutical compatibility of the components. The solution to this problem lies both in implementation of sufficient training of medical specialists and in the possibility of obtaining all necessary information using available information and analytical systems. It also shows the results of large-scale clinical studies of lipid-lowering therapy with statins, including the ones carried out in modern European guidelines for management of patients with acute coronary syndrome, including the use of endovascular myocardial revascularization. It has been shown that the optimal period for lipid-lowering therapy in acute coronary syndrome is the first hours of the disease development, and it is especially important to prescribe statins before percutaneous coronary interventions. The safety of lipid-lowering therapy, including high doses is reflected. Disputes about doses, statin choice in acute coronary syndrome are discussed. Sci., Professor; Kemerovo State Medical University; Scientific Research Institute of Complex Problems of Cardiovascular Diseases; olb61@mail.ru Annotation The aim of the research is to assess the efficiency and safety of early onset of physical trainings with individual selection of intensity in patients after coronary artery bypass grafting (CABG) in case of artificial blood circulation. Material and methods. 50 patients were included into the survey who were randomized into two groups depending on rehabilitation program. Patients of group 1 (n = 25, average age – 64 [58; 67]) had a course of early physical training with individual intensity selection; patients of group 2 (n = 25, average age – 62 [56; 68]) did not have any physical trainings. Selection of training intensity and evaluation of its effectiveness was carried out using the evaluation of spiro-cycle ergometry indicators. Results. On the 14th day after performing CABG, physical exercise tolerance (PET) increased in both groups from 50 W to 75 W. Classical indicators of cycle ergometry in 2 groups did not differ significantly, however, in group 1 there was a tendency (p = 0.06) to a lower value of heart rate (HR) at rest. Peak oxygen consumption on the 14th day after CABG was significantly higher (p = 0.04) in the group with physical trainings 14.2 [13.2; 15.4] versus 12.4 [11.3; 14.5] ml/ kg/min. No serious complications were recorded in any of the groups. The incidence of non-life-threatening complications did not differ in both groups. Conclusion. The early inclusion of physical training with individual intensity selection to the rehabilitation program after CABG is a safe and effective way to increase the reserve capacity of cardiovascular and pulmonary systems. Material and methods. The study involved 58 patients with β - thalassemia major, receiving blood transfusion and chelating therapy at the Republican Thalassemia Centre in Baku. The patients were divided into 4 groups (І – aged from 3 to 5, ІІ – from 6 to 12, ІІІ – from 13 to 17 and ІV – from 18 to 35). The check group consisted of 16 people, without any somatic diseases (average age – 12.7 ± 2.0 years). Indicators of iron and calcium metabolism and cytokine status were determined by standard methods in all the patients. Results. The obtained data showed a significant (p <0.001) accumulation of excess iron in different age groups, despite the ongoing chelation therapy with desferal. There was a significant increase in blood hepcidin, which correlated with an increase in the activity of inflammatory cytokines (IL-6, IL-10, TNF– α) and an increase in the level of IL-2 in blood in all the age groups. There is negative correlation of low levels of parathyroid hormone with high levels of inflammatory cytokines, as well as negative correlation level of parathormone with hepcidin (ρ ranged from - 0.36 to - 0.53 in different age groups). Conclusion. The predominance of anti-inflammatory component in activated cytokine cascades contributes to the demarcation of local inflammatory processes, including the ones in the oral cavity, being the most important factor in life prolongation in general in patients with β – thalassemia major. The revealed age dynamics of metabolic disorders increase, developing in β-thalassemia major, is associated with the progressive morbidity of patients with concomitant pathology. DOI 10.20333/2500136-2018-6-48-52 Authors Annotation The aim of the research is to evaluate the features of laboratory parameters, characterizing the manifestations of urinary syndrome and changes in the hepatobiliary system in children with severe intestinal infections. Material and methods. 61 children (35 boys and 26 girls) with acute intestinal infections aged from 2 months to 5 years were examined. A complex of general clinical and biochemical blood examinations were carried out, excrement examination was carried out bacteriologically and by means of PCR. Statistical processing was performed using the software package StatSoftStatistica 6.1. Results. In all the age groups of the examined children, there were pathological changes in the urinary syndrome in the form of high frequency proteinuria, the increase of creatinine level in every second child in groups aged under 1 year and from 1 to 3 years. Pathological changes in biochemical blood analysis showed the signs of cytolytic syndrome: significantly elevated levels of ALT and ALP were recorded in children group aged from 1 to 3 years, the level of AcAT in children under 1 year (p<0.05); on average, in groups, elevated levels of LDH, ALT, and ALP were significantly higher in children from 1 to 3 years; elevated level of AcAT in the group of children under 1 year (p<0.05). Conclusion. High proteinuria frequency may indicate the development of nephropathy in children with severe intestinal infections, and the increase of creatinine level does not exclude the acute kidney damage. Cytolytic syndrome signs indicate abnormal liver function. DOI 10.20333/2500136-2018-6-53-57 Authors Annotation The aim of the research is estimation of sensitivity to antibiotics of Escherichia coli community-acquired isolates in the Sakha Republic (Yakutia) according to AMRmap data. Material and methods. The data for 2014-2016 from online platform AMRmap was used in order to assess the sensitivity of Escherichia coli isolates to antimicrobials. Results. The proportion of isolates of the Enterobacteriaceae family among the 42 microorganisms found in the Sakha Republic (Yakutia) with communityacquired urinary tract infections was 95 % (n = 40). In 26 cases out of 40 (65 %) it was Escherichia Coli, in 9 (23 %) – Klebsiella pneumonia, in 4 (10 %) – Enterobacter cloacae. 69-89 % of Escherichia Coli isolates were resistant to penicillin preparations, including the ones with beta-lactamase inhibitor (ticarcillinclavulanate, amoxicillin-clavulanate). The activity in this group was maintained only in piperacillin-tazobactam (94 %). From 50 to 69 % of Escherichia coli isolates were resistant to preparations from the group of cephalosporins of the III-V generation without beta-lactamase inhibitors addition. High antimicrobial activity was observed only in the III generation of cephalosporin with beta-lactamase inhibitor, ceftazidime-avibactam (100 %). 62 % of the studied isolates were resistant to fluoroquinolones. Preparation resistance to aztreonam from the monobactam group was 62 %. Aminoglycoside resistance ranged from 4 to 23 %. Escherichia coli isolates showed sensitivity to preparations from the group of carbapenems, glycylcyclines and fosfomycin (100 %). Conclusion. The results of the study indicate that antibiotic resistance in the Sakha Republic (Yakutia) is a problem, both for the present and the future periods, and requires the adoption of immediate measures taken by regional healthcare authorities. Voino-Yasenetsky Krasnoyarsk State Medical University; vpefremova@mail.ru Annotation The aim of the research is determination of stomach arteries and veins capacity and their venoarterial index at different age periods. Material and methods. The study was conducted on stomach preparations of 80 male corpses. Inclusion criteria: male corpses of the I period of mature age (n = 20), elderly (n = 30) and senile (n = 30) ages; absence of pathology of the studied organ. Exclusion criteria: a known cause of death connected with the pathology or injury of the desired internal organs; macro- and microscopic internal organs pathology signs detected during material sampling. Histological preparations were done according to the standard histological method with the subsequent determination of arterial and venous vessels capacity (Kernogan index) and the capacitive characteristics of vascular bed (venoarterial index) using the appropriate formulas. Results. Arteries and veins capacity (Kernogan index) was determined and the capacitive characteristics of the organ’s vascular bed (venoarterial index) were evaluated in the first period of mature age, which is commonly referred as period of biological stability. The regularities of the age variability of these parameters in older age groups are established. It is noted that since age, Kernogan index of arterial stomach vessels and venoarterial index tend to increase, while the venous vessels capacity slightly decreases. The obtained data indicate the adaptive capabilities of blood stomach vessels in specific functioning conditions at postnatal ontogenesis stages. Conclusion.The obtained data are quite consistent, confirming and supplementing the available information on the nature of age-related transformations of the blood vessels of the internal organs. Material and methods. The method was applied to 18 patients brought with a clinical picture of gastrointestinal bleeding by an ambulance. The study was performed in operating or resuscitation rooms, in the presence of anesthesiologist-resuscitator in case of allergic reaction or the need for emergency surgical treatment. Fluorescence and narrow-spectrum imaging were used, along with indocyanine green solution (Germany) (0.125 mg / ml water) - 40 ml, high-frequency surgical device “BOWAAKC 350 PLUS” (Germany). Results. Bleeding sources: chronic gastric and duodenal ulcers – 8 cases (44.5 %), Mallory-Weiss syndrome was detected in 6 patients (33.3 %), acute gastric ulcer – 4 patients (22.2 %). In 10 patients (55.5 %) bleeding activity was regarded as Forrest IA. In 8 cases (44.5 %) – as Forrest IB. For the first time in Novosibirsk, endoscopic visualization of bleeding source by means of indocyanine green solution (Germany) with subsequent haemostasis was applied. In all patients with intensive bleeding, there was good visualization of the bleeding source. Clear visualization of bleeding source during washing the mucous membrane with indocyanine green, combined with infrared light mode examination, was obtained due to differences in blood colour glow of the one present in the stomach lumen, that is coloured by the preparation and the fresh one coming from the bleeding source. Conclusion. The first experience of using indocyanine green in emergency endoscopy in surgical patients was positive. It is advisable to study it further with subsequent analysis of the obtained results, specification of technique recommendations and spread of clinical application range. As a result of complex treatment, vertebral column deformity is eliminated, stable fixation is achieved, and there noted a regression of neurological disorders that the patient had before the operation. Material and methods. The following methods are used in the work: content analysis of scientific literature and regulatory documents; structural and organizational analysis; statistical method; data copying method. The subject of research is information support of the insured citizens about the possibility of health examination. Results. According to the results of the study, it was revealed that the actual procedure for the participants interaction in compulsory health insurance, with information support of the insured citizens at the stage of organizing regional health examination, does not correspond to the regulatory Rules. Organizational problems, affecting the efficiency of the information interaction process are identified. When analysing the report forms about the information support of the insured citizens about the possibility of undergoing health examination, there was revealed an insufficient resonance of those who underwent health examination. Statistical coefficients are determined, confirming the relationship between the number of the insured people individually informed about the possibility of undergoing health examination and the number of the insured who underwent health examination from the number of the informed. When analysing the appeals of citizens to the rights protection department for the insured, the reasons for the appeals were determined, the main ones among which are: the need to obtain information on the procedure for undergoing health examination of the adult population. Conclusion. The obtained data indicate a high demand for the population information support, which affects the efficiency of organizing and conducting health examination of certain groups of the adult population. DOI 10.20333/2500136-2018-6-92-97 Authors Annotation The aim of the research is a comparative analysis of the epidemiological data on syphilis in Novosibirsk city from 2007 to 2016. Material and methods. Retrospective statistical analysis was performed, using the reporting forms of state statistics No. 9 “Information on the morbidity of sexually transmitted infections and infectious skin diseases” and No. 34 “Information on patients with sexually transmitted diseases and infectious skin diseases” of federal and regional observation, approved by Goskomstat of the Russian Federation. Results. Over the past decade, there has been a decrease in syphilis morbidity in Novosibirsk city by more than 2.7 times. The early latent form prevails in general pathology structure (in 2007, 67.7 per 100 thousand, in 2016, 24.9 per 100 thousand of population), over the past period, there is a decrease in the morbidity of primary (20.1 % – 2007, 4.3 % – 2016) and secondary (32.3 % – 2007, 11.4 % – 2016) forms of syphilis, however, the number of registered cases of neurosyphilis increase (0.7 % – 2012 3.1 % – 2016) as well as of latent unspecified form of syphilis (21.3 % – 2012, 38.3 % – 2016). Males dominate (from 50.6 % in 2009 to 57 % in 2016) in sexual structure of syphilis morbidity. Single patients with congenital syphilis were registered in the period from 2008 to 2013. Conclusion. Over the past decade, there has been a tendency towards the decrease in syphilis morbidity, however, compared with the data for the Russian Federation, Novosibirsk city has higher rates. The day when professor S. I. Zalesskiy gave the first lecture to students of medical faculty is considered the date of chemistry department foundation. Later the department was subdivided into general chemistry department and medical chemistry department. The particular attention is paid to the scientific work carried out at chemistry departments in different time periods. It is worth noting that since its foundation until nowadays the scientific work at chemistry department has been devoted both to organic synthesis and to the study of properties of natural waters, therapeutic mud, peat, for their further use in medical purposes. Since the 70-s of the 20-th century, a student research group has been working and actively developing on the basis of chemistry department. Senior students, as well as first-year students are able to participate in scientific work of the department, in various conferences and competitions. Scientific work of students is headed by qualified tutors. In the 21-st century, chemistry department continues the traditions of classical academic education, and besides introduces new innovative educational technologies. Professional and fundamental training of future specialists is the main goal of chemistry department.
