Th is article is a scientifi c review of the literature data for the past 20 years concerning cytokine signal suppressor (SOCS5) gene and epidermal growth factor receptor gene (EGFR), based on OMIM, PubMed, and NCBI databases. SOCS5 and EGFR genes action mechanisms, the structure and functioning of the encoded data by protein genes are considered. Th e results of the published studies, confi rming the role of SOCS5 and EGFR genes in the development of pathology of lungs and other organs are presented. These genes association with the development of allergic bronchial asthma is shown by several case of studies.

Th e aim of the research. Assessment of correlation between the concentration of infl ammation markers, as well as N-terminal fragment of natriuretic peptide (NT-proBNP) and diff usion capacity of lungs (DLCO) in patients with myocardial infarction (MI).
Material and methods. Th e study includes 107 patients with myocardial infarction, 88 of whom (82.2 %) are men. All patients had I grade of cardiac
insuffi ciency by Killip, Q-forming MI developed in 93 (86.9 %) people. Coronarangiography was done in all patients (100%), percutaneous coronary
intervention in 100 (93.4 %) people. Diff usivity of the lungs and the level of infl ammation markers, as well as NT-proBNP in the blood serum were examined in all the patients on the 10-14th day.
Results. Depending on the level of DLCO, two groups have been formed: Group 1 – 59 (55.1 %) patients with normal DLCO, Group 2 – 48 (44.9 %) patients with a decreased DLCO data. In the group with the decreased DLCO, the level of C-reactive protein (CRP) was signifi cantly higher – 2.6 (1.9, 4.6) vs. 3.8 (3.3, 8.2), p <0.001. According to linear regression analysis, a signifi cant correlation of CRP and DLCO levels in patients with MI has been revealed. As a result of multiple linear regression analysis, it was revealed that the level of serum CRP concentration is able to explain 11 % of diff usion capacity of lungs in patients with MI. Nonlinear interconnection between concentration of tumor necrosis factor α (TNF-α) and DLCO level was noted.
Conclusion. Th e association of CRP as universal infl ammation marker with violation of ACM conductivity has been revealed, which confi rms the involvement of infl ammation in the development of diff usion disorders in patients with MI. Th e obtained quadratic model confi rms the inconsistent data on the infl uence of TNF-α level on ACM conductivity

The aim of the research. Assessment of bleeding frequency in diff erent age groups. Search and development of eff ective tools for such complications
forecasting.
Material and methods. 492 patients, hospitalized with STEMI to Kemerovo Cardiovascular Clinical Hospital, were included into the present singlecenter
register observatory investigation. Two groups, based on hemorrhagic complications, were formed. Th e bleeding frequency in patients of this
sampling is estimated. All major predictors of hemorrhagic complications are calculated. The model of “2STEPS” scale is constructed, its validity
is estimated.
Results. The bleeding frequency was 42 (8.53 %) cases (21 case – puncture hematomas, 11 –pulsating hematomas, 10 – gastrointestinal bleeding). The frequency of “heavy” bleeding in patients of diff erent age groups was identical. Hemorrhagic complication predictors: female gender and signs of acute heart failure are revealed. A prognostic model of bleeding risk assessment scale in patients with STEMI was created. Th e validity of the developed scale has been tested in comparison with the existing ACTION and REACH scales, moreover, high predictive level has been obtained.
Conclusion. Th e developed 2STEPS scale is an eff ective, accurate and easy-to-use tool for predicting bleeding in STEMI, which makes possible to use it widely in clinical practice.

The aim of the research is to study the functional state of the left and right ventricles in patients with complete blockade of the right and left legs of the bunch of Gis with idiopathic lesion of the conduction system of the heart.
Material and methods. 72 patients with idiopathic blockade of the left (LBBB) and right (RBBB) leg of a bunch of Gis were examined. All patients underwent routine echocardiography with determination of basic parameters of hemodynamics, as well as indices of myocardium strain, left ventricular (LV) twist, as well as anatomic and functional parameters of the right ventricle (RV).
Results. In idiopathic LBBB group, compared to the group with RBBB, the global longitudinal strain parameters (-12.7 ± 5.4 % and -19.3 ± 3.1 %, p = 0.0001), circular strain on the basal (-7.4 ± 3.2 % and -19.3 ± 4.5 %, p = 0.0011) and at the apical levels (-5.0 ± 3.0 % and -20.2 ± 6.7 %, p = 0.0003), as well as the resulting LV twist (6.7 ± 3.6º and 12.5 ± 6.9º, p = 0.0001) are less. When assessing the anatomic and functional parameters of RV in LBBB group, compared to RBBB, there was a larger size of outfl ow tract (26.8 ± 3.7 mm and 21.9 ± 2.7 mm, p = 0.0001, respectively), the thickness of the free RV wall (6,7 ± 0,9 mm and 5,8 ± 0,7 mm, р = 0,0013), smaller values of systolic displacement of tricuspid valve ring (22.9 ± 4.9 mm and 26.0 ± 5, 6 mm, p = 0.0214) and peak basal КМ segment velocity (12.9 ± 2.7 cm / sec and 15.0 ± 4.0 cm / sec, p = 0.0391).
Conclusion. Violation of intraventricular conduction along the left leg of a bunch of Gis is accompanied by a decrease in myocardium strain properties and LV twist parameter. Th e result of LV dysfunction is the changes in RV work, connected with the compensatory increase in the load

Th e aim of the research is to assess the neurofunctional state of the brain by means of electroencephalography and neuropsychological examination of
children with chronic cerebral ischemia caused by intra- and extracranial pathology of the main arteries with cerebral circulation disturbance of ischemic type in anamnesis.
Material and methods. 31 children with chronic cerebral ischemia, caused by the pathology of the main arteries of the brain got surgical treatment in Scientific Research Institute of Neurosurgery aft er academician N.N. Burdenko for the period from 2012 to 2017. Th e age of children varied from 2 to 18 years. All the patients underwent comprehensive clinical and instrumental examination aimed at clarifying the leading pathogenetic mechanism of the disease. Special attention was paid to the examinations, clarifying the structural and functional features of the pathology of the cerebral vascular system (USDG, TKDG, spiral CT angiography, MRA, brain perfusion examination, examination of brain blood fl ow with functional tests). In order to assess functional state of the brain, in addition to standard neurologic examination, EEG and neuropsychological examination were done. As far as, the last two kinds of examinations in children with vascular pathology were not practically carried out in other institutions, we decided to share the details of the results in this article.
Results. Assessing neurofunctional state of the brain of children with chronic cerebral ischemia, the children were divided into two groups. Comparison of
EEG-examination data and neuropsychological diagnostics was performed. In the group of children with intracranial pathology of the main vessels, more
severely expressed pathological changes are revealed, which is manifested both in the nature of changes in the electroencephalogram and in neuropsychological diagnostics.
Conclusion. By comparing the data of these two methods against the background of diff use pathological changes, it was possible to single out the specifi city of the brain in children with extra- and intracranial pathology of the main vessels in the neurofunctional state.

The aim of the research is to analyze the course of intranatal and early neonatal periods in children with CP combined with perinatal CNS damage.
Material and methods. The article presents data of anamnesis with the features of early neonatal and intranatal periods course in 81 children with CP: 50 children with hypoxic-ischemic damage of CNS (HID CNS) and 31 children with hypoxic-hemorrhagic damage of CNS (HHD CNS), including
neurosonography data assessment.
Results. Th e anamnesis of children with CP was studied, boys accounted 59.3 %, girls – 40.7 %, and the total number of premature infants was 87.7 %. It was found out that children suff ering from CP were born predominantly by cesarean section –79%. Severe degree of asphyxia at 1 minute was registered in most newborns (60.5 %), infants with average degree of asphyxia accounted 34.6 %. Most of the children (97.5 %) underwent mechanical ventilation. In the group with HHD CNS in 16,1 % a prolonged ventilation was performed (for more than 20 days), in the group with HID CNS no cases were recorded.
Conclusion. Taking into consideration high percentage of premature infants’ birth, it is necessary to improve obstetric and gynecological control of pregnant women and to diagnose fetal pathology, to improve the methods of resuscitation of newborns with asphyxia. Timely neurosonography in the early neonatal period provides opportunities for early diagnosis and further prognosis with the identifi cation of risk groups for CP.

The aim of the research is to study the frequency of distribution of allelic variants of polymorphisms of cytokine genes IL17A (rs2275913), IL17F (rs763780) in psoriasis and psoriatic arthritis in Caucasians of Krasnoyarsk city in order to identify psoriatic disease progression markers.
Material and methods. 176 patients with vulgar psoriasis in Caucasians, living in Krasnoyarsk, aged 18 to 66, were examined. Patient groups were formed
in the following way: psoriasis (n = 77), psoriatic arthritis (n = 99). Th e check group consisted of 102 practically healthy volunteers. Venous blood served as a material for the study. DNA isolation was performed using a standard sorbent method. Th e allelic variants of cytokine genes IL17A (rs2275913), IL17F (rs763780) were determined by restriction analysis of amplifi cation products (RFLP analysis) of specific genome parts. Diff erences in the frequency of genotypes in patients and check groups were carried out using Pearson χ2 criterion. The correlation of the disease with genotypes was assessed by calculating the chances ratio and the confi dence interval.
Results. Based on the conducted studies, it was found out that in psoriasis the frequency of genotypes AG and GG of polymorphism IL17A (rs2275913)
statistically is signifi cantly higher in comparison with psoriatic arthritis. Genotypes AG and GG of polymorphic variant rs763780 of IL17F gene statistically are signifi cantly more frequent in patients with psoriatic arthritis in comparison to check ones.
Conclusion. Th e result of the study has shown that in patients with psoriasis the frequency of genotypes AG and GG of polymorphism rs2275913 of IL17A
gene statistically is signifi cantly higher in comparison with psoriatic arthritis, which indicates its correlation with the risk of psoriasis. In addition, genotypes AG and GG IL17F (rs763780) are more common in patients with psoriatic arthritis, being genetic markers of disease developing risk. Th e obtained findings confirm that Th 17-lymphocytes and polymorphism of IL17A / F genes play an important role in the pathogenesis of psoriasis and psoriatic arthritis.

Th e aim of the research is to analyze the results of diff erentiated surgical treatment of patients with degenerative diseases of lumbar spine with the use of puncture techniques.
Material and methods. The study included 841 patients who, aft er conducting puncture provocative techniques, were implemented with a diff erentiated surgical approach: in isolated discogenic pain syndrome (n = 215), puncture posterolateral laser nucleoplasty was performed; in isolated facet syndrome (n = 247) laser denervation of facet joint took place; in verifi cation of combined clinical symptomatology (n = 379), a simultaneous operative intervention on intervertebral disc and facet joint was performed. To assess the result of surgical treatment, the level of pain by visual analogue scale (VAS) in the lumbar and lower extremities and the functional status according to ODI were studied, the remote satisfaction with the operation on Macnab scale and the development of perioperative complications were analyzed. 
Results. A signifi cant decrease in the severity of pain syndrome in lumbar spine and lower limbs was found, respectively: in I group from 61 mm (59; 72) to 16 mm (13; 17) (p = 0.001) and from 72 mm (65.84) to 17 mm (13; 20) (p = 0.003); in II group from 74 mm (63; 85) to 15 mm (12; 17) (p = 0.005) and from 62 mm (57; 69) to 16 mm (13 (P = 0.002), and from 76 mm (67, 89) to 14 mm (12; 17); and in III group, from 73 mm (64; 80) to 15.5 mm; 17) (p = 0.005); positive dynamics of functional status: in I group from 82 (76, 86) to 16 (12; 18) (p = 0.007), in II group from 72 (68; 76) to 16 (14; 18) (p = 0.003) , in III group from 82 (80, 84) to 14 (12; 18) (p = 0.004); by Macnab subjective evaluation scale, in 18 months, there were predominantly excellent and good results in group I 107 (49.76 %) and 88 (40.93 %), respectively, in group II–111 (44.93 %) and 94 (38, 05 %) of cases, respectively, in group III–169 (44.59 %) and 133 (35.09 %) cases, respectively.
Conclusion. Diff erential surgical tactics, that have been chosen, according to VAS, allowed achieving in the early and distant postoperative periods a minimal level of pain syndrome in the lumbar and lower extremities, good functional status according to ODI and high degree of patient’s satisfaction with the operation performed without any perioperative complications.

Th e aim of the research. Comparison of affi nity of human bioactive factors (VEGF, bFGF / FGF-2 and SDF-1α) to the corresponding human and ovine
receptors (VEGFR2, FGFR1, FGFR2, CXCR4).
Material and methods. PDB numbers of tertiary structures of the mentioned proteins, as well as UniProtKB-numbers of their amino acid sequences, were
determined from the RCSB PDB, PDBe and PDBj databases. Further, by aligning and comparing the amino acid sequences in Geneious program, the homology of primary and secondary structure of human and ovine proteins were analyzed. Since the tertiary structure of the analyzed proteins was experimentally decoded only for humans, homologous modeling of tertiary structure of corresponding ovine proteins was performed in SwissModel program. Molecular docking analysis was carried out in PatchDock program in order to evaluate ligand affi nity to the receptor and in ZDOCK program to construct 3D models of ligand-receptor interactions.
Results. Primary and secondary structure of the analyzed human and ovine proteins was identical for more than 90 %, that made it possible to suggest
similar affi nity of bioactive factors to relevant receptors. The percentage of diff erences between the conditional indices of the affi nity of human bioactive factors and ovine receptors in comparison with relevant conditional indices of affi nity for human receptors was only slightly higher than the combined percentage of interspecifi c diff erences in primary structure of these proteins. 3D modeling of interactions of diff erent variants of bioactive factors with their receptors has shown that the variant of deciphering the tertiary structure of bioactive factor (PDB-number) practically does not aff ect their interaction with receptors within a single biological species.
Conclusion. Affi nity of human bioactive factors (VEGF, bFGF, SDF-1α) to the corresponding ovine and human receptors (VEGFR2, FGFR1, FGFR2, CXCR4)
differs insignificantly

The aim of the research. Assessment of compliance with the requirements of regulatory legal acts in terms of providing paid medical services in state and
non-state medical organizations.
Material and methods. To assess the implementation of the rules for providing paid medical services, a sociological survey was conducted among 220 health workers, 20 experts in the fi eld of healthcare organization and 350 consumers of paid medical services. Using calculating the relative risk procedure, the influence of professional and social characteristics of health workers on observation of rules of providing paid medical services has been studied.
Results. Along with annual growth in the volume of medical services provided on onerous basis, violations of consumer rights are being registered, the main cause of which is non-observation of paid medical services with the requirements of legislation of the Russian Federation in the fi eld of private healthcare sphere by providers. According to the results of sociological survey, the main violations of the requirements of regulatory legal acts in the sphere of providing paid medical services were revealed, among them: non-observance of the procedure for providing information to consumers on a fee-for-service basis, lack of keeping primary medical documentation and statistical accounting forms, violation of the procedure for making contract for providing paid medical services, etc.
Conclusion. The conducted study allowed to determine the most common violations of the procedure for providing paid medical services, as well as to
identify certain professional and social characteristics that are the risk factors for violating the rules for proving paid medical services and, as a consequence, leading to violation of consumers’ rights for services on onerous basis

The aim of the research. Assessment of teachers’ competence in the development of healthy lifestyle of the population.
Material and methods. In order to study additional opportunities aimed at forming healthy lifestyle, a sociological survey of school teachers was conducted to determine their willingness to participate in the process of forming healthy lifestyle of the population.
Results. Th e majority of respondents (64.6 ± 4.2 %) consider it necessary and appropriate to conduct work with population, aimed at formation of healthy lifestyle; 23.8 ± 3.7 % partially share this viewpoint and only 2.1 ± 1.3% of the respondents do not consider it necessary and appropriate to conduct primary medical prevention among population. Most teachers consider it advisable to receive training on healthy lifestyle issues. Th e most convenient forms of improving their competence in matters of healthy lifestyles the surveyed teachers consider: training in regional centers of medical prevention or health centers. Most of the interviewed teachers believe that greatest part of population has the need to obtain information on healthy lifestyle issues. Taking into consideration the level of population’s informatization, the respondents noted the advisability of providing people with information materials on healthy lifestyle on the Internet.
Conclusion. As a result of the conducted sociological research it is found out that teachers consider it advisable to carry out preventive work with population, they are ready to become competent in matters of healthy lifestyle and to participate in creating information materials on healthy lifestyle issues.

A clinical case of congenital heart disease – an abnormal origin of the left coronary artery from the pulmonary artery (Bland-White-Garland syndrome), is revealed for the fi rst time in a 15- year-old girl. This congenital heart disease in natural course and without surgical correction is characterized
by high mortality (up to 90 % of cases) in the fi rst year of life, being connected with the development of extensive myocardial infarction, or multiple heart attacks, but when a network of intercoronary anastomoses develops, it cannot appear clinically for a long time. Classic signs of this patient’s disease were revealed during echocardiography, being confi rmed by coronary angiography, which allowed timely surgical intervention.

Th e possibilities of diagnosing infectious endocarditis signifi cantly expanded aft er the creation of standardized clinical diagnostic criteria and after
the development of visualization technologies, such as echocardiography. However, the sensitivity of transthoracic echocardiography is only 40-60%, while transesophageal echocardiography is 90-100%. Th is article describes a clinical case of complexity of diagnosing infectious endocarditis with false-negative results of primary transthoracic echocardiography.

Idiopathic pulmonary fi brosis (IPF) is a progressive fi brotic lung disease, the cause of which is unknown. It is characterized by a progressive decrease of pulmonary function, and by unfavorable prognosis in case of specific treatment absence. Further goes the description of a clinical case of
idiopathic pulmonary fi brosis combined with emphysema with a positive clinical and radiological response for the prescribed antifi brotic – nintedanib.

The aim of the research. Analysis of the work of stroke prevention room located in consultative and diagnostic clinic of the Regional Clinical Hospital.
Material and methods. Th e subject of the study was 149 patients with a stroke who were discharged from the regional vascular center in Krasnoyarsk. Dynamic monitoring of patients for 6 months with clinical and neurological examination, Rankin, Rivermid scales, USA scale of NIHSS National Institute of Health, telephone interview method were used. Th e statistical analysis was carried out using the SPSS package.
Results. Aft er being discharged from the hospital, only 73.39 % from total number of patients referred to stroke prevention room. High risk of recurrent
stroke was found in 38.26 % of patients, among vascular risk factors hypertension prevailed 97 %, atrium fi brillation – 23.48 %, diabetes –8.72 %, cerebral atherosclerosis – 88.59 %. Hemodynamically signifi cant stenoses were in 11.41 % of patients. Th e advisability of monitoring in stroke prevention rooms in the first three months aft er the stroke was noted, during which, the early specialized dispensary observation, is the most eff ective in developing individual strategies for secondary stroke prevention, including selection of carotid endarterectomy, stenting of carotid arteries. In future, in case of stable patients’ status, regular medical check-up is continued in polyclinic at the place of residence.

Gornitskaya (Zuckerstein-Gornitskaya). She was one of the famous pediatricians of the country, the fi rst woman to head Children’s Hospital named aft er K.A. Raukhfus, she became the fi rst female professor of pediatrics and the head of the department in Leningrad. In the history of Krasnoyarsk Medical Institute, Edda Abramovna Gornitskaya was the first professor, who headed the department of children’s diseases; she was the organizer of children’s doctors society, she was consulting and organizing the work during the severe years of the Great Patriotic War.
