Voyno-Yasenetsky Krasnoyarsk State Medical University; allasalmina@mail.ru Annotation Alzheimer's disease (AD) is a chronic neurodegenerative disorder, initial symptoms of which are manifested typically in 8-10 years before the diagnosis of dementia, and is characterized by progressive memory loss and cognitive impairment. Pathological signs of AD include the accumulation of extracellular senile plaques and intracellular neurofibrillar tangles. Today, AD is an incurable disease with enormous social and economic consequences, and insufficient study of molecular events that underlie in the base of the development of this pathology. The experimental AD model, used in scientific research, aimed at solving a wide variety of tasks, such as searching for molecules - targets, identifying risk factors for disease development, preclinical and clinical studies of pharmaceutical products, etc., thus contributing to the explanation of the complex interactions that occur on molecular and cellular levels and, consequently, a better understanding of AD ethiopathogenesis, as well as a therapeutic effective screening. This scientific review of literature are discussed various experimental AD models are identified and analyzed their advantages and disadvantages. To study carrier frequency I / D (rs4646994) of polymorphism of the ACE gene, associated with secondary auricle fibrillation form (AF) to create risk patients groups that prone to develop of this disease. Materials and methods. According to the study design were examined 50 probands with secondary AF and 77 persons of their families I, II, III degree of relatives. These families make up the main group of our research. The control group was formed from a relatively healthy people without cardiovascular disease history. This group consisted of 100 persons. All patients were examined by the following survey methods: medical history and complaints, an electrocardiogram, XM ECG, ECHO CS, VEM, CHPSLP, molecular genetic analysis, patients who have AF, were studied thyroid hormones. Results. We have produced a statistically significant prevalence of carriers homozygous genotype II for common allele among probands with secondary OP (34,0% ± 6,7) compared with the control group (14,0% ± 3,5), p = 0.004. It was also found a statistically significant prevalence of carriers homozygous of DD genotype for the rare allele among control subjects (36,0% ± 4,8) compared to probands with secondary OP (10,0% ± 4,2), p = 0.001. It was found statistically significant prevalence of carriers homozygous genotype II probands among women with secondary OP (40,6% ± 8,7) compared to the control group of women (17,2% ± 4,7), p = 0.012. Also found a statistically significant prevalence of homozygous carriers of DD genotype among the control group women (35,9% ± 6,0) compared with women of the main group (3,1% ± 3,1), p = 0.0005. Conclusion. Thus, according to the results of the study found a statistically significant prevalence of carriers homozygous genotype II for common allele of ACE gene among probands with secondary OP and in the subgroup of women compared with those of control group. But at the same time there is a statistically significant prevalence of homozygous carriers of DD genotype for the rare allele in the control group, compared with probands with secondary OP and a subgroup of women. Obtained data indicate multifactorial genesis of AF and confirm that homozygous carriers of DD genotype has conditionally protective effect against the development of AF. To study the influence of piribedil (pronoran) to the state of cognitive functions in patients with coronary heart disease after coronary bypass surgery. Materials and methods. It were studed 64 patients with coronary heart disease. AH patients were divided into two groups. In the main group (n = 32) in addition to the therapy was prescribed pronoran (piribedil) at a dose of 50 mg once daily for 3 months. The comparison group (n = 32), patients in the postoperative period received standard therapy. There were assessed cognitive function in patients before surgery, on the 10th day, after three and six months after coronary bypass surgery. The article presents a case of a patient rehabilitation with coronary heart disease, mild cognitive disorders after coronary bypass surgery. Results. In the group of patients during therapy with piribedil in 6 months after coronary artery bypass were noted positive dynamics in almost all tests. Conclusion. Taking into account the authentic improvement the results detected in patients during treatment with piribedilom we can with high probability to assume a positive effect of this drug on memory indexes and attention, psychomotor speed processes. To evaluate the clinical course of nonalcoholic fatty liver disease (NAFLD) in men and women and to study the features of the lipid metabolism disorders. Materials and methods. The study involved 164 patients with nonalcoholic fatty liver disease, including 103 men and 61 women. Results. Severe hypercholesterolemia (more than 7.8 mmol / l) was registered more often at men. Hypertriglyceridemia and hypoalfacholesterinemia were more pronounced in men, that indicating more severe disorders of lipid metabolism. Insulin resistance (IR) was significantly higher in men. Conclusion. Studies suggest that NAFLD in men and women with equal frequency is detected on the background of metabolic syndrome (MS). However, a cluster of factors MS is significantly more pronounced in men. The main pathophysiological factors contributing to the formation of NAFLD, namely, IR and dyslipidemia were more significant in men due to the earlier development of hepatic steatosis in males and younger age compared to women. To assess the types and structure forms of the maxillary sinuses in Krasnoyarsk population according to the cone-beam computed tomography. Materials and methods. At 150 cone-beam computed tomograms (SVST) were studied 300 maxillary sinuses of the population of Krasnoyarsk and the Krasnoyarsk Region, obtained by the tomograph «Picasso Trio» with EzImplant program (South Korea). One hundred and fifty sinuses of right maxillary sinus and 150 left maxilla were estimated by the type of pneumatization, the shape of the structure and symmetrically arranged to use the data for planning the endodontic treatment of molars of the upper jaw. Results. At men and women with almost equal frequency was prevailed hyper pneumatized type of structure of the maxillary sinuses without statistical difference by gender and location. Symmetry of the types of sinuses was observed in 146 (97.3%) of 150 SBST. The prevailing form of sinus of right and left upper jaws was triangular: 70.2% and 70.2% in men, 70.7% and 70.9% - women. Conclusion. Type, shape and location of the sinus of the upper jaws, the ratio of the tops of the roots to the sinus bottom determines the effect of odontogenic foci of inflammation to the state of its mucous membrane and must be taken into account in the planning and evaluation of the results of endodontic treatment of teeth. To analyze the organization of care for COPD patients in outpatient and inpatient settings in the Krasnoyarsk Region. Materials and methods. There were analyzed the information on the fact of treatment the adult population (18 years and older) by code ICD J44 in dynamics for the period from 2009 to 2014. The volume and structure of health care was evaluated, its share in the outpatient and inpatient conditions with day and day and night stay. Results. The article provides the basic aspects of legal regulation of medical care to patients with COPD in the Krasnoyarsk Region. According to our data, in the period from 2009 to 2014 the outpatient and inpatient care was provided to an average of 29 thousand persons annually. Most of the treatment cases were carried out in outpatient conditions. However, for the six analyzed years the trend has changed in favor of increasing the number of admissions to the day and night hospitals. Conclusion. During the study period it was noted a decrease in the length of stay of patients with COPD in a hospital. To reveal the ways to bring health activity centers for children (CPC) in accordance to their purpose - the prevention of chronic noninfectious diseases in the population of the Siberian Federal District (SFD) from childhood. Materials and methods. Based on the submitted to the MH RF by health centers for children of the age-specific data, in contrast to the official reporting form No. 68, graduation of childhood period proposed by the ICD by 10th revision, was given the characteristics of covered contingent and the results of inspections. The paper summarizes the data on 30 CZD operating in the Central Federal District for 2013. The represented absolute values were entered in an Excel database and transferred to the relative, based on the average child population in the Federal District at the beginning of 2013. The obtained data was analyzed by methods of descriptive statistics and the SWOT-analysis. Results. Generalized information of the SFD was compared with data of the country, was represented the spread of indicators in the district subjects, that allowed to make a number of conclusions and proposals for the development of the regulatory base, within framework the activity of CZD is carrying out at the moment. Conclusion. For a more effective analysis of CZD it is necessary to improve the system of registration the recording and reporting indicators. As part of the solutions to these problems it is necessary to develop a system of versatile objective assessment of the efficiency of these units. To study clinical and hematological changes in infants with infectious mononucleosis caused by the Epstein-Barr virus in the acute phase of the disease. Materials and methods. It were examined 72 children with infectious mononucleosis caused by the Epstein-Barr virus in the acute phase of the disease in ages of 6 months to 12 months. General blood test was performed on the hematology analyzer «CELL-DYN Ruby» (Abbott, USA). Atypical mononuclear cells in peripheral blood was studied by light microscopy. The content of total bilirubin and its fractions in blood serum, the activity of enzymes ALT, AST, and thymol were determined by conventional biochemical methods. Results. All observed patients had acute beginning of the disease. Tonsillopharyngitis phenomena were in all of the observed children, while in 77.77% (56 persons) tonsillitis cases had lacunar character. Generalized lymphadenopathy was detected in 86.11% of the patients, hepatomegaly - in 100%, splenomegaly in 86.11% of cases. Every second child with severe disease has infectious cardiopathy. The hemogram showed an increase the number of white blood cells and the absolute number of lymphocytes, as well as increasing the content of atypical mononuclear cells in peripheral blood and increased ESR. Leaving the hospital 1/3 patients has manifestations of generalized lymphadenopathy and hepatomegaly, in 16.66% of patients there were detected atypical mononuclear cells in the peripheral blood of 4% to 8%, almost in every second child (47.22%) at the end of inpatient treatment the percentage of lymphocytes in the peripheral blood exceeded Conclusion. In 2/3 of the patients at the time of discharge from the hospital were kept separate clinical manifestations and changes in the hemogram. The study revealed the low interest of young professionals working in the medical institutions of Krasnoyarsk Region in obtaining qualification category, compared with young professionals in Krasnoyarsk and Norilsk.
