Voyno-Yasenetsky Krasnoyarsk State Medical University; tzamay@yandex.ru Annotation The aim of the research. To evaluate the role of the energy status of the tumor cells in the process of its development. Materials and methods. As the object of study were used the cells of Ehrlich ascites carcinoma. Functional state of the tumor was evaluated by the mitotic index and number ofapoptotic and necrotic cells in it. Energy state of the tumor was determined by the Pi / fl-NTPratio. Results. It was revealed that during tumor growth it was observed the phase changing the number of tumor cells in it – the rapid growth of the tumor was replaced by a sharp decline in the number of tumor cells, and then equally increasing its number, with a gradual beginning of the terminal phase. The rate of oxygen consumption by the ascites cells in different phases of Ehrlich carcinoma growth remained practically unchanged. The energy state of the cells is gradually deteriorating, and then restored. Conclusion. Energy state of ascites cells is not a factor determining the rate of growth of the Ehrlich ascites carcinoma in conditions in vivo. The main role in the regulation of tumor growth belongs to imbalance between antagonistic signaling pathways engaged balance between cell death and cell division processes, normally providing the strict maintenance of the required number of cells in the tissue. The objective of this study was to investigate change in calcium maintenance of Ehrlich ascite adenocarcinoma cells in the course of tumor growth. Materials and methods. Cell viability was determined by counting apoptotic and necrotic cells under the fluorescence microscope. Intracellular calcium concentration was determined with the fluorescent probe 2,5 mM Fura-2AM. Relative microviscosity of membranes was estimated by fluorescence method using Pyrene. The activity of Ca-ATPase was assessed by the amount of inorganic phosphate produced during ATP hydrolysis. Results and discussions. We revealed that dynamics of Ehrlich ascite adenocarcinoma growth in our experiments was different from the commonly observed one. We noticed one peculiarity. The guantity of cells decreased on the day 12 after the tumor transplantation. This diminution was caused by mitotic activity lowering and by increase of apoptotic cell quantity the day before. Ehrlich ascite adenocarcinoma growth was accompanied by change in intacellular calcium maintenance which was independent of Ca-ATPase and cell membrane microviscosity. Conclusions. Thus, our experiments have shown the change in calcium concentration. Apparently Ca2+ activated proliferation. However calcium maintenance was settled byROS. Possibly ROS supression in tumour on the day 12 was able to put down proliferative processes stimulation. To evaluate the efficacy and safety of pharmacoinvasive strategy compared to pPCI, conducted in the early stages of acute myocardial infarction with ST-segment elevation. Materials and methods. It was analyzed 816 cases of patients with STEMI. All patients received reperfusion therapy. 455 patients were conducted pPCI and 361 got prehospital thrombolytic therapy (TLT), followed by early PCI. Patients in the case ofCAG within 60 minutes from the initial medical contact were not included in the study. Key time intervals were comparable between the groups. Results. FIS was accompanied by a significant decrease in mortality as compared to a total pPCI in general group (3.6% and 9.7%, respectively). Mortality in the group older than 75 years old was comparable (17.5% at pPCI and 16.7% at the FIS; p = 0.616). The main predictor of hospital mortality was the lack of distal blood how to the IZA (TIMI - 0-1) in the primary angiography, as well as the severity of coronary lesions on the Syntax score above 22 points. Conclusion. Implementation of PCI in the early stages of the TLB exceeds pPCI strategy in patients younger than 75 years old, and is not associated with an increase the number of hemorrhagic complications. To study the state of the respiratory system, systemic inflammation parameters and their effect on the structural and functional parameters of the left and right parts of the heart in patients with bronchial asthma. Materials and methods. It was examined 244 people: 204 patients with bronchial asthma (BA) in acute and in the dynamics in 12 months and 40 healthy individuals. Parameters of the function of external respiration were recorded on the unit of general plethysmography «ErichEger» (Germany). Determination of cytokines TNF-a, IL-2, IL-4, IL-6, sCD31 / sPECAM-1 in peripheral blood plasma gas been done by the method of IF A (eBioscience, USA). Results. In all groups during exacerbation it was determined significant increase of concentration IL-6, TNF-a, IL-2, IL-4, in comparison to healthy persons. In patients with asthma of varying severity was observed increase of sCD31 / sPECAM-1 in peripheral blood compared to control (p <0,05), regardless of the observation period. We have found that endothelial dysfunction is linked with a change in the geometry of the left atrium, which confirms the positive correlation relationship between the content of sCD31 in blood plasma and the diameter of the left atrium in the group of patients with steroid-dependent asthma (SZBA) in acute (r = 0,49, p = 0, 03). Positive correlations are established between the content of TNF-a and IL-4 and OTSLZH index (r=0,44, p = 0,01; r= 0,68, p = 0,044, respectively). Structural indicators of the right ventricle in patients with asthma were comparable to the control (p> 0,05). However, with asthma severity were recorded a decrease in theE/A PZh in patients the 2nd and 3rd groups in acute and in SZBA and in dynamics in 12 months (p <0,05). Conclusion. In the formation of functional disorders of the heart muscle in patients with asthma, a significant role played the level of bronchial obstruction, systemic inflammation and endothelial dysfunction that must be considered when drawing up the plan of examination and treatment. To study the features of structural and functional changes in the cardiovascular system in male patients suffering from arterial hypertension (AH) combined with chronic obstructive pulmonary disease (COPD). Materials and methods. There were examined 89 male patients with COPD II-IVstages, the average age - 62.0 [56,5- 69,5]. The control group included 27male patients with essential hypertension, the average age - 60.0 [58,0-64,5]. Status of intracardiac hemodynamics, structural and functional parameters of the heart were evaluated by echocardiography. Results. Taking into account the existing pulmonary hypertension in patients with COPD, there is an increase in total lung resistance (RL) and due to an increase in total peripheral vascular resistance (SVR), and more pronounced in subjects with comorbidity of these pathologies, which leads to a change in pulmonary blood flow and load increase to the right departments of heart. More marked changes the left heart were observed in patients with isolated hypertension and COPD combined with hypertension. Comparing two groups of patients with COPD, significant results were obtained by the thickness of the interventricular septum (IVS) and posterior wall of the left ventricle (PWLV), which were more pronounced in patients in group 2 (p <0,05), which is due to the development of hypertension. In examined patients with COPD in combination with hypertension was more common type of prognostically unfavorable remodeling - concentric left ventricular hypertrophy (LVH), also often determined «restrictive» diastolic dysfunction of the right ventricle (DDRV) 76,7% (p = 0,03) and diastolic heart dysfunction (70 % overall), which is the result of complex diastolic disorders and subsequent hemodynamic changes. Conclusion. In patients with COPD in combination with hypertension prognostically unfavorable type of remodeling is more often - concentric left ventricular hypertrophy (LVH) is determined by «restrictive» DDRV -76.7% and diastolic dysfunction of the heart as a whole -70%. To develop the measures aimed at improving the results of treatment of gunshot wounds of the neck in patients from the civilian population, obtained under the conditions of local military conflicts. Materials and methods. The base of the study is a retrospective analysis of 129 cases of gunshot wounds of neck. Age of the wounded ranged from 18 to 65 years old. All the wounds were received during the local military operations in the Republic of Chechnya in the period from 1991 to 2000. Results. The analysis within the next postoperative period in patients with gunshot wounds of the neck it was revealed that it has complications in 51.9% of cases, main of them were purulent-septic -21.7%, which developed in 21.7% of cases. Conclusion. The development of such a large number of septic complications can be connected to several factors, firstly, patients didn't receive gualified medical care in prehospital phase. Secondly, in the hospital, working in extreme conditions of war, there was no enough eguipment, which oftenleads to diagnostic errors. The third factor, which is also to be considered is the severity of gunshot wounds themselves. To identify the markers of a healthy human body to assess its somatoneuropsychological status. Materials and methods. There were examined representatives of youthful age: men - 1064 persons, women - 1569. Methods: anthropometry according to the classical method of V. V. Bunak, calculation of indices to assess the physical status (body mass index, the index of sexual dimorphism byJ. Tanner), bioimpedance analysis of body composition, electroencephalography. Results. On the basis of the study there were obtained the results, proving the possibility of using some anthropometric, bioimped- ansometric parameters and electroencephalographic indicators as the somatoneuropsychological health markers. Conclusion. There were conducted a complex examination of representatives of youthful age that made possible to distinguish the following group of markers to assess somatoneuropsychological status, which can be viewed as predictors of socially significant diseases: body mass index, the component composition of the body, namely the higher values of fat mass, index of sexual dimorphism, biological age and the ratio the rate of aging, the phase angle, the rhythm of the higher nervous activity and the coefficient of inter-hemispheric coherency. Myotonic phenomenon is a symptom of neuromuscular disorders, characterized by slow relaxation (prolonged contracture) of skeletal muscles after voluntary contraction or electrical stimulation. Myotonia may involve all groups of muscles. However, the nature of muscle destruction could vary depending upon the particular disease.
