CLINICAL AND PATHOGENETIC ASPECTS OF CARDIORENAL RELATIONSHIPS ON BACKGROUND OF TYPE 2 DIABETES Pages: 5-13 Evseeva M.V., Kalaeva Y.Y. Clinical and pathogenetic aspects of cardiorenal relationships on background of type 2 diabetes. Siberian medical review. 2016; № 2: 5-13. DOI 10.20333/25000136-2016-2-5-13 Authors Evseeva Maria Vladimirovna Research Associate of the Blood Circulation Pathology Laboratory; Federal State Budgetary Scientific Institution Research Institute for Complex Issues of Cardiovascular Diseases; m_evseeva_2016@mail.ru Kalaeva Victoria Vladimirovna Cardiologist; Federal State Budgetary Scientific Institution Research Institute for Complex Issues of Cardiovascular Diseases; viken-k-86.86@mail.ru Annotation Violation of the kidney filtration function is common enough among patients with cardiovascular disease (CVD), especially against disorders of carbohydrate metabolism (IGM). Renal dysfunction (PD) is associated with more frequent development of cardiovascular complications. In turn, the probability of development of renal disease in patients with cardiovascular disease is significantly higher than in general population. This bidirectional pathogenetic mechanisms existing between cardiovascular and kidney diseases were the basis for the formation of cardiorenal syndrome concept. The review discusses the general parts of pathogenesis within the cardiovascular continuum. In addition, it shows the effect on metabolic disorders during CVD combined with renal dysfunction. The review reveals a recent view on the main mechanisms of cardiorenal syndrome. Keywords: cardiorenal syndrome, systemic inflammation, endothelial dysfunction, insulin resistance Reference List: 1. To evaluate twisting of the left ventricle (LV) on the background of left bundle branch block (LBBBj in patients c cardiomyopathies and study of the hemodynamic conseguences of possible changes. Materials and methods. The study involved 22 patients with ischemic and dilated cardiomyopathy with left ventricular ejection fraction less than 40%, divided into two groups: physiological counter-rotating basal and top and abnormal unidirectional. Results. Although there are no differences in terms, characterizing cardiac remodeling, in the group with abnormal rotation occurred substantially smaller twisting indicators and changes were observed in the flow of the pulmonary veins that are typical of high end-diastolic pressure LV. In group of patients with abnormal rotation systolic left atrial filling fraction was 32,3 ± 8,07%, whereas in the group with a physiological counterrotating -53,1 ± 10,1 (P = 0.000226). A possible cause of these changes are great indicators of intraventricular dyssynchrony in the group with abnormal rotation. Time interval delay redaction of the rear wall was 63,3 ± 35,1 ms in comparison with persons with normal left ventricular rotation - 8,0 ± 17,9 ms (p = 0,015922), reflecting the greater degree of mechanical dyssynchrony. Conclusion. Thus, LBBB has a complex negative influence on the electrical activation and contraction of IV, the result of which was a violation of twisting, mechanical dyssynchrony and deterioration of diastolic function. To study the immunological parameters and neural network evaluation of its informativity in patients with active acromegaly and in a state of remission. Materials and methods. The study involved 106 patients with acromegaly: 88 people at the stage of active acromegaly and 18 patients with remission of acromegaly. The method of indirect immunofluorescence and by immunosorbent assay were investigated immunological parameters of blood. Evaluation of information was carried out with the help of neural network classifier Panalyzer 4,0. Results. There were found the changes, dependent on the stage of the disease in the cellular part of the immune system, which are determined by the number of subpopulations of T-lymphocytes and the level of cells, expressing the activation molecules. Changes in concentrations of immunoglobulins in serum of acromegalic patients don't depend on the stage of the disease. The most informative indices of neural network classifier were the percentage of helper and effector subpopulation T-lymphocytes and cells with expressing the activation receptors. Conclusion. Features of functioning immune system with active acromegaly are to develop the state of hyperactivity of the immune system, which is characterized by increased activation of the cellular component of the immune system in an imbalance of humoral immunity. In remission acromegaly has the trend towards normalization of indexes of cellular immunity, but while high level of B-lymphocytes and NK-cells and by increasing the number of cells expressing and CD95-CD25- receptors. Using neural network classifier was confirmed high informativity of lymphocytes expressing activation markers and the number of helper and effector T cells in the immunopathogenesis of acromegaly. It was done the evaluation of the treatment results of 17 patients operated on for ruptured arterial brain aneurysm in the acute period of subarachnoid hemorrhage, depending on the severity of the initial state. Patients were divided into two groups depending on the severity of vasospasm: I group - 10 patients according TCD 3-5 hours MCA blood flow speed was less than 120 cm / sec (10 observations in the age of 50,1 ± 8,3 years); Group II - 7 patients 3-5 th day from the moment of development on the AGR CAA flow rates were more than 120 cm / s (age 55.7 ± 6.5 years). Were evaluated: the level of consciousness of oppression and the severity of the NAO on the scale (GCS, Hunt-Hess, Fisher) at the time of admission; the level of the final neurological deficit extended Glasgow Outcome Scale (GOS extended); and conducted monitoring of cerebral blood flow velocity by TCD. Results. It was established that during the development of vasospasm with increasing speeds of more than 120 cm / s on admission there is a statistically significantly higher rate of blood flow in the intracerebral arteries, carrying a causal aneurysm; and the previously observed increase and a greater flow velocity, which corresponds to a more pronounced vasoconstriction. It was determined that the initial assessment on the scale of Hunt-Hess, Fischer, GCS reflects a greater severity of SAH in the group with increased blood velocity by intracerebral arteries. Conclusion. It was found that the adverse outcomes of SAH (death - severe disability) are characteristic for patients with the development of vasospasm.
