Currently, despite the successes and progress in medicine, incidence of preterm births (PB) does not decrease in the world. Moreover, there is an increasing tendency in industrialized countries. Modern approach for PB prevention is based on incomplete understanding of its occurrence mechanisms, as well as sequence and timing of preventive measures. Primarily obstetricians and gynecologists focus their attention on identifying PB risk factors, as well as on cervical shortening, while prevention includes prescribing medications of progestogen group and / or methods of mechanical action on obturator cervical function. Effectiveness of the currently existing methods of PR prevention are analyzed in the review.

Ultrasound lungs examination has been considered uninformative for decades due to the high acoustic resistance between soft tissues and airy lungs. Recently, the work in emergency conditions has proved the usefulness of ultrasound technologies, especially while diagnosing acute respiratory failures. Introduction of ultrasound technologies into the practice of intensive care units made it possible to change the ‘philosophy’ of diagnosing critical conditions, promoting ultrasound as primary technique, even in the era of widespread use of computed tomography. Portable ultrasound diagnostic devices, along with general clinical examination, made it possible to differentiate rapidly such acute conditions as cardiogenic shock, acute respiratory distress syndrome, pneumothorax, atelectasis, and other lung lesions

The paper analyzes modern literature on methods of cognitive functions recovery as dual tasks, implying simultaneous execution of motor and cognitive tasks. There is ambiguity of currently obtained data on the effectiveness of dual tasks in cognitive rehabilitation of elderly people, people with traumatic and neurodegenerative brain damage. It is assumed that the success of dual tasks is associated with the severity of transfer and interference effects. Opportunities and prospects of using dual tasks in cognitive rehabilitation of patients after cardiac surgery are outlined.

Patients with myocardial infarction (MI) and atrial fibrillation (AF), the number of which is progressively increasing every year, make up a high-risk group for both recurrent cardiovascular events and bleeding; they require special attention from clinicians. The literature review provides data on features of pathogenesis and clinical manifestations of MI in patients with AF. The analysis of data on AF effect observational studies on short-term and long-term prognosis in patients with myocardial infarction was carried out. Mechanisms of occurrence, clinical features and prognostic value of postinfarction AF are considered. From the standpoint of modern clinical guidelines, information is presented on features of MI invasive treatment in combination with AF. Algorithms of anticoagulant and antiarrhythmic therapy in patients of this category are considered.

Considering the tendency for the increasing number of elderly people, the problem of improving life quality of this category of population become more relevant. Senile asthenia (“frailty”) is one of the key clinical conditions that is currently the most common and age-associated (geriatric syndromes) one leading to loss of autonomy, increased hospitalization, as well as to disability and mortality in elderly people. The article reviews domestic and foreign literature data on issues related to the peculiarities of this condition, tactics of managing patients with SAS, as well as existing measures to prevent the development and progression of “frailty” in elderly patients

Nowadays, there is an increasing necessity to use a lot of medical products (MP) in one patient. This can lead to potential nephrotoxic adverse reactions (ADRs) among drugs. One of drug-induced kidney damage manifestations is acute tubular necrosis (ATN), which causes the development of severe complications associated with the use of hemodialysis and a high risk of mortality. The purpose of this review is to analyze literature data concerning groups of drugs and individual drugs, the intake of which is associated with the development of drug-induced ATN, risk factors, prevention and treatment methods. Among drug-induced ATN development risk factors are both general, associated with the development of drug-induced acute kidney damage (for example, age, history of chronic kidney disease, etc.), and specific ones, typical for certain groups of drugs. Most often, drug-induced ATN develops while taking antibacterial, antiviral and anticancer drugs. The main method of treatment for drug-induced ATN is abolition of nephrotoxic drugs, but if it is impossible, then there are certain patient management schemes. To prevent and to diagnose drug-induced ATN at early stages, that is the key for favorable prognosis, it is necessary to aware of doctors of different specialties about such pharmacotherapy complications.

Tactics features for managing out-patients with cardiovascular comorbidity in Krasnoyarsk city

The aim of the research is to study the structure of cardiovascular continuum within comorbid pathology within outpatient clinic of Krasnoyarsk; to evaluate the quality of out-patients management in accordance with modern clinical guidelines. Material and methods. To conduct a single retrospective analysis, 1928 medical records of Krasnoyarsk cardiovascular out-patients were taken. All the patients consulted local therapists in 2018. Differences were statistically significant at p <0.05. Results. Comorbid pathology is observed in patients with CVD in 87% of cases, and its growth is noted with age. It was revealed that ischemic heart disease (CHD) and obesity are associated with type 2 diabetes mellitus (DM-2). When using antihypertensive therapy, 43.2% of patients achieved blood pressure (BP) values <140/80 mm Hg. In 46.8% of cases, patients who were prescribed statins had total cholesterol (TC) values <5.0 mmol / L. Only 18.5% of patients with DM-2, following lipid-lowering therapy had a total cholesterol level <4.5 mmol / L, that was also registered only in 15.9% of patients with chronic kidney disease (CKD). Heart rate (HR) <80 beats / min was observed in 75% of patients taking drugs of b-blocker group. Conclusion. Cardiovascular comorbidity among patients is widespread and its rates are evidenced to increase with age. Antihypertensive, lipid-lowering and antithrombotic therapy do not fully comply with current clinical guidelines. It is necessary to individualize the approach to patients with combined pathology; to provide comprehensive diagnosis, treatment, and to inform the population on primary and secondary prevention adequately.

The aim of the research is to assess the prevalence of traditional and behavioral risk factors for cardiovascular diseases (CVD) among urban and rural residents of large industrial Siberian region, depending on education level. Material and methods. The study included 1600 people, aged 35-70, who are permanent residents of Kemerovo City and Kemerovo region. Considering the living conditions, two groups were formed. The first group included 1109 (69.3%) respondents living in the city and 491 (30.7%) respondents from countryside. The study analyzed such traditional and behavioral risk factors for CVD as body mass index (BMI), glucose levels, lipid metabolism (total cholesterol, lipoprotein cholesterol of high and low density (LCLD), triglycerides), blood pressure, tobacco and alcohol consumption, etc. According to education level, the respondents were subdivided into the following categories: people with school education (primary education, secondary education), people with secondary specialized education (vocational school, technical schools, colleges) and people with higher education (higher educational institution). Results. Average BMI values (30.3 (26.1; 34.6) versus 28.4 (24.9; 32.3)) kg / m2, p<0.001), systolic (135 (121.5; 151.0 ) versus 131.5 (119.0; 147.0)) mm Hg, p = 0.005) and diastolic blood pressure (89 (80.5; 97.5) versus 86.5 (78.0; 95, 0)) mm Hg, p<0.001), LCLD (3.7 (2.9; 4.4) versus 3.5 (2.8; 4.2)) mmol / L, p = 0.008) were statistically significantly higher among people living in rural areas than among urban citizens. Higher education was associated with BMI decrease by 1.2 kg / m2 (p = 0.002), systolic blood pressure by 3.2 mm Hg (p = 0.002), and diastolic blood pressure by 2.1 mm Hg (p = 0.001). Moreover, people with such education were characterized by a decreased risk not only of obesity (OR = 0.6, 95% CI [0.5-0.8], p <0.001), but also of arterial hypertension (OR = 0.7 , 95% CI [0.5-1.0], p <0.001), hypertriglyceridemia (OR = 0.72, CI [0.53-0.98], p = 0.037), as well as by an increased risk of developing high level of LDL (OR = 1.36, CI [1.02-1.82], p = 0.038). When conducting a logistic regression analysis, after leveling the influence of gender and age, it was found that smoking was associated with lack of higher education (OR = 0.6, 95% CI: 0.48-0.79, p <0.001). Conclusion. Regardless of place of residence, maximum prevalence of obesity, arterial hypertension and diabetes mellitus was observed among people with secondary vocational education, while alcohol consumption was observed among people with higher education. Higher education was associated with a reduced risk of obesity, arterial hypertension, hypertriglyceridemia, smoking, and an increased risk of developing high levels of low-density lipoprotein

The aim of the research is analysis of frequency and nature of adverse reactions during the use of drugs in the Republic of Sakha (Yakutia) according to automated information system of Russian Federal Service for Surveillance in Healthcare “Pharmacovigilance” (Roszdravnadzor “Pharmakonadzor”). Material and methods. Data from the AIS Roszdravnadzor “Pharmakonadzor” in the Republic of Sakha (Yakutia) for the period from 2014 to 2019 were used for the analysis. It should be noted that since April 1, 2019 data collection is carried out using the modified notification form. Results. For the period of 2014-2019 300 reports (292 primary and 8 repeated) on adverse reactions and cases of therapeutic ineffectiveness of drugs were registered. According to the leading criterion concerning person’s life severity, there were notifications on 20 deaths of patients, 60 life-threatening cases, 1 case of disability, 105 cases of hospitalization or prolongation of its terms, 35 clinically significant event cases. It was noted, that mainly, antimicrobial drugs and drugs for nervous system treatment caused adverse reactions. Conclusion. Report frequency on drugs adverse reactions or lack of therapeutic effect in the region was assessed as insufficient. There should be additional training for medical workers on the procedure for implementing pharmacovigilance in medical organizations, correct notification filling, which ensures reports completeness and its quality

The aim of the research is to develop and implement a protocol for accelerated rehabilitation in patients with operable colorectal cancer and solitary lung metastasis; to evaluate the effectiveness of the protocol at all stages of perioperative period with simultaneous surgical treatment. Material and methods. The results of treating patients with colorectal cancer and synchronous lung oligo-metastasis were analyzed. All patients underwent simultaneous surgical treatment. In the study group, perioperative support was carried out using the principles of accelerated rehabilitation; in retrospective check group patients were treated according to traditional methods. In postoperative period, perioperative parameters, number and severity of complications were assessed. Results. Within the study, no statistically significant differences were found in the groups, both in the duration of surgery and in the amount of blood loss. The largest number of complications in both groups are of I and II degrees and there were no static differences. However, faster recovery of gastrointestinal function, as well as shorter in-patient period was observed in the study group. Conclusion. The study showed the feasibility of simultaneous surgical treatment in this category of patients. Moreover the use of accelerated rehabilitation protocol can reduce the duration of postoperative in-patient period, as well as decrease of gastrointestinal tract function recovery time.

A clinical family case of Jervell and Lange-Nielsen syndrome in 2 children is presented. Long QT syndrome is one of the leading causes of sudden cardiac death in children. Jervell and Lange-Nielsen syndrome is one of the most severe and rare types of long QT syndrome. Symptoms of the disease appear in infancy; they are characterized by a lengthening of QT interval on ECG, syncope as a result of life-threatening ventricular tachycardia and ventricular fibrillation, combined with congenital bilateral hearing loss.

The aim of the work is to study clinical cases and outcomes of pregnancy in women who have pelvic fracture during pregnancy. The article examines two cases of concomitant injury in pregnant patients at 25-37 weeks. In both cases, there is a break of pelvic bones of various localization. On basis of these observations, the authors draw conclusions about the significance of the mother’s condition and associated injuries for a favorable outcome.

Analysis of doctors of different specialties staffing in the Russian Federation and abroad

The aim of the research is to analyze the indicators of medical personnel of various specialties staffing in the Russian Federation and abroad. Material and methods. The data from Rosstat in the field of healthcare were used. The proportion of doctors of various specialties was calculated. Descriptive statistics method was used; indicators trend analysis was carried out. Results. The indicator of staffing by doctors of all specialties per 10,000 population in the studied period had a downward trend (-4.3%). The largest decrease occurred among pediatricians (-36.0%). Less decrease concerns obstetricians and gynecologists (-1.7%), psychiatrists and psychiatrists and narcologists (-11.7%), phthisiatricians (-16.6%). An increase was registered among therapeutists (except GP) (+ 1.8%), (family) GP (+ 14.2%), surgeons except anesthesiologists and resuscitators (+ 4.0%), X-ray specialists and radiologists (+15, 3%), and dentists (+ 2.3%). The indicator was stable among ophthalmologists, otorhinolaryngologists, neurologists, dermato-venereologists, and exercise therapy doctors. The proportion of therapeutists (except GPs) and GPs tended to increase and amounted 1.7% and 22.8%, respectively, by the end of the studied period. The indicator increased among surgeons except anesthesiologists and resuscitators, obstetricians and gynecologists, ophthalmologists, otorhinolaryngologists, neurologists, X-ray specialists and radiologists, exercise therapy doctors, and dentists. The decrease was registered in pediatricians, psychiatrists and psychiatrists and narcologists, phthisiatricians, dermato-venereologists. The Russian Federation occupies a leading position compared to foreign countries in terms of staffing by doctors (per 100,000 population). Conclusion. To implement personnel policy in healthcare, it is necessary to pay attention not only to quantitative indicators providing population with doctors, but to qualitative ones as well. It is necessary to balance the ratio of doctors of different specialties to form a sustainable national health system. Scientifically based mechanism for specialist’s distribution should be developed.

The aim of the research. In connection with the adoption of new measures to limit alcohol consumption, from May to June 2020, specialists of health monitoring department of Krasnoyarsk Regional Center for Public Health and Medical Prevention conducted an online survey to study the attitude of both residents of the region and representatives of executive authorities to measures taken by the state to reduce the level of drunkenness in Russia. Material and methods. The article is based on the results of online survey. The survey involved 641 respondents over 18 years old, while 166 were representatives of local government. Results. It turned out that, in general, the population does not have negative attitude towards government restrictions on alcohol sale. However, some peculiarities of respondents’ attitude to government measures for the restriction of alcohol sales were revealed. It related to respondents’ gender and age, the fact of alcohol consumption and the status of the respondents’ influence on society. Conclusion. The study revealed the attitude peculiarities of region residents and representatives of regions territory administrations to restriction of alcohol sale taken by the state authorities. The study also included comparison of respondents’ opinions on restrictive measures depending on gender and age and the fact of alcohol consumption
